The subject of the public procurement is the supply of reagents and consumables, a closed system and microtubes for blood collection for the Clinical Laboratory Department at UMHAT “Sveta Anna” - Sofia AD upon prior request, in accordance with the specification attached to the documentation for participation in the procedure. The contract performance period is 24 (twenty-four) months, with periodic deliveries depending on the needs of the Contracting Authority. The delivery period shall be proposed by the participant in calendar days and shall not exceed 5 calendar days from receipt of a written request from the Contracting Authority. The procedure covers 25 separate lots, including sub-items, and tenders may be submitted collectively for all separate lots or for each one individually. A participant may tender for a separate lot provided that it covers absolutely all sub-items within that separate lot. The participant shall submit only one tender, in accordance with the conditions specified in the documentation for participation, and the submission of variants is not permitted. The reagents and consumables offered must comply with the parameters set out in the specification prepared by the Contracting Authority and must be fully compatible with the respective device. When participating for separate lot No. 19, sub-items No. 19.1, No. 19.2, No. 19.3, No. 19.5, separate lot No. 20, sub-items No. 20.1, No. 20.2, No. 20.3 and No. 20.4, participants must submit samples of the consumables and reagents offered, demonstrating the conformity of the offered products with the parameters set out in the technical specification. The samples must be provided in sealed opaque packaging bearing the following markings: name of the participant, correspondence address, telephone number, fax number if possible, and email address, the name of the procurement and the separate lots and nomenclature units (sub-items) for which the samples are submitted. The samples shall be delivered to the “Legal Services and Public Procurement” Department, floor 2, Administrative Building of the medical facility, Sofia, 1 Dimitar Mollov St. In its tender, the participant shall specify the incoming registration number under which the samples were registered in the Contracting Authority's registry office, or, where they were sent by postal or other courier service, the shipment number. Where the samples are submitted after the tender, the tender number shall also be indicated on the packaging. The public session for opening the submitted samples will be held on the date on which the tenders are opened, at 14:30, in the “Legal Services and Public Procurement” Department, floor 2, Administrative Building of the medical facility. The persons who may attend the public session are the participants' legal representatives or persons authorized by them. When participating for separate lots No. 1, 2, 3, 4, 5, 6, 7, 8, 9, 10, 11, 12, 13, 14, 15, 16, 17, 18, 23, 24 and 25, the participant must declare the full compatibility of the consumables and reagents offered with the respective device. Each participant must attach to its technical proposal the following documents, which form an integral part thereof – a Letter of Authorization, valid as of the tender submission date, from the manufacturer or its representative or authorized person, issued in the participant's name and granting the participant the right to trade in its products. Where the Letter of Authorization is issued by a representative or authorized person of the manufacturer, a document from the manufacturer certifying the representative authority of those persons/companies must also be submitted. Separate lot 1 contains 46 sub-items, in accordance with the quantitative specification attached to the documentation for participation in the procedure. Separate lot 3 contains 245 sub-items, in accordance with the quantitative specification attached to the documentation for participation in the procedure. Separate lot 8 contains 170 sub-items, in accordance with the quantitative specification attached to the documentation for participation in the procedure. Separate lot 16 contains 10 sub-items, in accordance with the quantitative specification attached to the documentation for participation in the procedure. Separate lot 18 contains 2 sub-items, in accordance with the quantitative specification attached to the documentation for participation in the procedure. Separate lot 20 contains 14 sub-items, in accordance with the quantitative specification attached to the documentation for participation in the procedure. Separate lot 21 contains 13 sub-items, in accordance with the quantitative specification attached to the documentation for participation in the procedure. Separate lot 23 contains 33 sub-items, in accordance with the quantitative specification attached to the documentation for participation in the procedure.
