Opportunity description

SUBJECT MATTER OF THE CONTRACT 1. The subject matter of the contract is the purchase, together with delivery at the request of the Contracting Authority, of medicinal products entitled: “SUPPLY OF MEDICAL DEVICES PART III – DRESSING MATERIALS AND NONWOVEN PRODUCTS”, described in detail in Annex No. 2 to the SWZ – Assortment and Price Form and in Annex No. 6 to the SWZ – Draft Contractual Provisions. 2. The subject matter of the contract has been divided into 21 parts, hereinafter referred to as: “Packages”, described in detail in Annex No. 2 to the SWZ – Assortment and Price Form. 3. The Contracting Authority permits the submission of partial tenders, where a part (package) comprises: Package No. 1 – gauze dressings, cotton wool Package No. 2 – plaster bandages, undercast padding Package No. 3 – adhesive tapes, adhesive dressings Package No. 4 – non-sterile gauze compresses Package No. 5 – sterile gauze compresses Package No. 6 – sterile adhesive dressings Package No. 7 – surgical films Package No. 8 – surgical films with an antibacterial agent Package No. 9 – adult incontinence briefs Package No. 10 – haemostatic dressings Package No. 11 – neurosurgical patties Package No. 12 – fixation tapes Package No. 13 – dressings for puncture sites, central line insertion sites Package No. 14 – specialist dressings Package No. 15 – silver dressings, wound-cleansing dressings Package No. 16 – haemostatic dressings with human fibrinogen and thrombin Package No. 17 – sterile compresses with an X-ray-detectable thread Package No. 18 – specialist dressings with LC technology Package No. 19 – hydrocolloid dressings Package No. 20 – patient shorts Package No. 21 – waterproof sheets. 4. Parts (packages) may not be divided by Contractors; tenders that do not contain the full scope of the subject matter of the contract specified in the partial task will be rejected. 5. The Contractor may submit a tender for all parts of the contract. 6. The Contracting Authority does not limit the number of parts for which a contractor may submit a tender, nor does it specify the maximum number of parts that may be awarded to the same contractor. 7. A detailed description of the subject matter of the contract is provided in an annex to the SWZ, i.e. Annex No. 2 (assortment and price form). 8. The place of delivery and unloading is: - Hospital Pharmacy of the S.P. Wojewódzki Szpital Chirurgii Urazowej, ul. Bytomska 62, 41-940 Piekary Śl, or the off-site warehouse of the Hospital Pharmacy of the S.P. Wojewódzki Szpital Chirurgii Urazowej located in Kochcice, ul. Zamkowa 3.
