The subject matter of the contract is the supply of antineoplastic agents, immunosuppressive agents and immunoglobulins, divided into 19 lots. The Contractor may submit a tender for any lot/lots or for the entire subject matter of the contract. The Contracting Authority does not permit the submission of variant tenders. The Contracting Authority – pursuant to Article 139 of the PPL – informs that it will first examine and evaluate the tenders in this public procurement procedure and will then perform the qualitative selection of the Contractor whose tender was rated highest, with regard to the absence of grounds for exclusion and fulfillment of the conditions for participation in the procedure. DOCUMENTS SUBMITTED WITH THE TENDER 1. The tender consists of: 1) a completed tender form, in accordance with Annex No. 1 to the SWZ; 2) a completed assortment and price form, in accordance with Annex No. 2 to the SWZ. 3) the declaration indicated in Chapter V, section 5, point 2) of the SWZ (if applicable). 2. Together with the tender, the Contractor is required to submit: 1) an extract or information from the National Court Register, the Central Register and Information on Economic Activity or another appropriate register – in order to confirm that the person acting on behalf of the Contractor is authorized to represent it. The Contractor is not required to submit these documents if the Contracting Authority can obtain them using free and publicly accessible databases, provided that the Contractor has indicated the data enabling access to these documents (original); 2) if a person whose authorization to represent the Contractor does not arise from the documents referred to in the preceding point acts on behalf of the Contractor: a power of attorney or another document confirming authorization to represent the Contractor; 3) a power of attorney to represent Contractors jointly applying for the award of the contract – if applicable; 4) if the bid security is provided in a form other than money – the original bid security document (guarantee or surety) – if applicable. 5) The declaration and documents indicated in Chapter VI A, to the extent specified therein as being submitted with the tender.
