Opportunity description

1. The subject matter of the contract is the purchase and successive delivery of oncology drugs and various drugs, specified in the lots, for the Fryderyk Chopin University Clinical Hospital in Rzeszów. 2. A detailed description of the subject matter of the contract and the Contracting Authority's requirements are set out in the Price and Tender Forms – Attachments No. 1 for the individual lots, constituting an integral part of the SWZ. 3. The Contracting Authority requires the offering of contract items that: 3.1. meet the requirements described in the Price and Tender Forms, 3.2. meet the requirements described in the SWZ, 3.3. are covered by the Act of 12 May 2011 on the reimbursement of medicinal products, foodstuffs intended for particular nutritional uses and medical devices – applies to medicinal products covered by reimbursement. 4. The scope of the contract includes the delivery of the contract items in the quantities specified in the Price and Tender Forms for the following period: - Lots No. 1 to No. 23 – 12 months 5. CONTRACT PERFORMANCE PERIOD: 5.1 Purchase and successive delivery of the contract items: Lots No. 1 to No. 23 - for 12 months from the date of conclusion of the contract. 5.2 For Lots No. 1 to No. 23, the Contracting Authority permits the possibility of changing the term of the contract concluded with the Contractor if the entire subject matter of the contract is not completed during its term. In such a case, the change to the contract requires the signing of an annex, provided that the Contracting Authority unilaterally notifies the Contractor of the proposed extension of the contract before the expiry of the period for which it was concluded. 6. Deadline for submitting a request for clarification of the content of the SWZ: 06.05.2026. 1. Oncology drugs: Mitoxantronum concentrate for solution for infusion, 2 mg/ml, 10 ml vial 2. The scope of the contract includes the delivery of the contract items in the quantities specified in the Price and Tender Form for the following period: - Lot No. 12 – 12 months. 4. The Contracting Authority does not permit the submission of equivalent tenders. 5. CONTRACT PERFORMANCE PERIOD: 5.1 Purchase and successive delivery of the contract items: Lot No. 12 - for 12 months from the date of conclusion of the contract. 5.2 For Lot No. 12, the Contracting Authority permits the possibility of changing the term of the contract concluded with the Contractor if the entire subject matter of the contract is not completed during its term. CONTRACT PERFORMANCE PERIOD: 5.1 Purchase and successive delivery of the contract items: Lot No. 21 - for 12 months from the date of conclusion of the contract. 5.2 For Lot No. 21, the Contracting Authority permits the possibility of changing the term of the contract concluded with the Contractor if the entire subject matter of the contract is not completed during its term.
