THE INDEPENDENT PUBLIC HEALTHCARE FACILITY, DISTRICT EMERGENCY MEDICAL SERVICE IN SOSNOWIEC, 41-200 SOSNOWIEC, ul. Completion of the visit must be confirmed by a written report. A scan of the report must be submitted together with the tender. If a site visit is not conducted for lots No. 1 and No. 2, the Contractor’s tender will be rejected pursuant to Article 226(1)(18) of the Public Procurement Law. Grounds for exclusion; A contractor shall be excluded from the procurement procedure in accordance with Article 108(1) and Article 109(1), points 1, 4, 5, 7, 8, 9 and 10 of the Public Procurement Law. The Contracting Authority shall exclude a Contractor from the procedure in the cases referred to in Article 5k of Council Regulation (EU) 833/2014 of 31.07.2014 concerning restrictive measures in view of Russia’s actions destabilising the situation in Ukraine (Official Journal of the EU No. L 229 of 31.7.2014, p. 1), as amended by Council Regulation (EU) 2025/2033 of 23.10.2025 amending Council Regulation (EU) 833/2014 concerning restrictive measures in view of Russia’s actions destabilising the situation in Ukraine (Official Journal of the EU No. L of 23.10.2025), i.e. a Contractor who is: 1. a Russian national, a natural person residing in Russia, or a legal person, entity or body established in Russia; . a legal person, entity or body whose ownership rights are directly or indirectly owned by more than 50% by a natural or legal person, entity or body, i.e. Russian nationals, natural persons residing in Russia, or legal persons, entities or bodies established in Russia; or 2. a natural or legal person, entity or body acting on behalf of or at the direction of: a natural or legal person, entity or body, i.e. − Russian nationals, natural persons residing in Russia, or legal persons, entities or bodies established in Russia, or − legal persons, entities or bodies whose ownership rights are directly or indirectly owned by more than 50% by a natural or legal person, entity or body, i.e. Russian nationals, natural persons residing in Russia, or legal persons, entities or bodies established in Russia. The grounds for exclusion under Article 5k of Council Regulation (EU) 833/2014, as amended by Council Regulation (EU) 2025/2033, also apply to subcontractors and suppliers where they account for more than 10% of the contract value. The Contractor is bound by the tender from the tender submission deadline until 08.08.2026 (88 days).
