INDEPENDENT PUBLIC HEALTHCARE ESTABLISHMENT DISTRICT EMERGENCY MEDICAL SERVICE IN SOSNOWIEC, 41-200 SOSNOWIEC, ul. Completion of the visit must be confirmed by a written record. A scan of the record must be submitted together with the tender. If the site visit for lots No 1 and No 2 is not carried out, the Contractor’s tender will be rejected pursuant to Article 226(1)(18) of the Public Procurement Law. Grounds for exclusion; A contractor shall be excluded from the contract award procedure in accordance with Article 108(1) and Article 109(1), points 1, 4, 5, 7, 8, 9 and 10 of the Public Procurement Law. The Contracting Authority shall exclude a Contractor from the procedure in the cases referred to in Article 5k of Council Regulation (EU) 833/2014 of 31.07.2014 concerning restrictive measures in view of Russia’s actions destabilising the situation in Ukraine (OJ EU L 229, 31.7.2014, p. 1), as amended by Council Regulation (EU) 2025/2033 of 23.10.2025 amending Council Regulation (EU) 833/2014 concerning restrictive measures in view of Russia’s actions destabilising the situation in Ukraine (OJ EU L of 23.10.2025), i.e. a Contractor that is: 1. a Russian national, a natural person residing in Russia, or a legal person, entity or body established in Russia; . a legal person, entity or body whose proprietary rights are directly or indirectly owned for more than 50% by a natural or legal person, entity or body, i.e. Russian nationals, natural persons residing in Russia, or legal persons, entities or bodies established in Russia; or 2. a natural or legal person, entity or body acting on behalf of or at the direction of: a natural or legal person, entity or body, i.e. − Russian nationals, natural persons residing in Russia, or legal persons, entities or bodies established in Russia, or − legal persons, entities or bodies whose proprietary rights are directly or indirectly owned for more than 50% by a natural or legal person, entity or body, i.e. Russian nationals, natural persons residing in Russia, or legal persons, entities or bodies established in Russia. The ground for exclusion under Article 5k of Council Regulation (EU) 833/2014, as amended by Council Regulation (EU) 2025/2033, also applies to subcontractors and suppliers where they account for more than 10% of the contract value. The Contractor is bound by the tender from the tender submission deadline until 08.08.2026 (88 days).
