I. The subject matter of the contract is the supply of IT equipment comprising laptop computers, desktop computers and computer accessories in connection with the implementation of the project entitled “Improvement of infrastructure and development of outpatient and day care at the Institute of Mother and Child” under FEnIKS. The Contracting Authority permits the submission of tenders for individual lots, divided into 3 lots. The Contracting Authority does not envisage awarding contracts referred to in Article 214(1)(8) of the Public Procurement Law. A detailed description and the method of performing the contract are provided in the product and price table, Appendix No. 2 to the SWZ, and the description of the subject matter of the contract – list of required parameters (“OPZ”), Appendix No. 3 to the SWZ, as well as the model contract, Appendix No. 5 to the SWZ. II. Pursuant to Article 13(1) and (2) and Article 14(1) and (2) of Regulation (EU) 2016/679 of the European Parliament and of the Council of 27 April 2016 on the protection of natural persons with regard to the processing of personal data and on the free movement of such data, and repealing Directive 95/46/EC (General Data Protection Regulation) (Official Journal of the EU L of 2016, No. 119, p. 1) (“GDPR”), we would like to provide information about the principles governing the processing of personal data where the Contracting Authority processes your personal data in connection with conducting a public procurement procedure. 2. The controller of your personal data is the Contracting Authority. 3. The Contracting Authority may be contacted regarding matters related to personal data protection: 1) by sending a letter to its registered office address or electronically by email to: dyr@imid.med.pl, or 2) via the ePUAP platform at: /IMiDWarszawa /SkrytkaESP. 4. The Contracting Authority has appointed a Data Protection Officer (“DPO”), whom you may contact regarding the processing of your personal data. The DPO may be contacted by email at: iod@imid.med.pl, by telephone at (22) 32 77 495, or in writing at the address of the Contracting Authority’s registered office. 5. The Contracting Authority processes your personal data for the following purposes: 1) conducting a public procurement procedure – pursuant to Article 6(1)(c) GDPR in conjunction with Article 9(2)(g) GDPR in conjunction with the provisions of the Public Procurement Law; 2) verification of information concerning the absence of criminal convictions as part of means of evidence relating to the entity – pursuant to Article 6(1)(c) in conjunction with Article 10 GDPR in conjunction with the provisions of the Public Procurement Law; 3) archiving personal data – pursuant to Article 6(1)(c) GDPR in conjunction with Article 9(2)(j) GDPR in conjunction with the Act on the National Archival Resources and Archives; 4) establishing, pursuing or defending claims in connection with the activities conducted – pursuant to Article 6(1)(e) GDPR or Article 9(2)(f) GDPR, i.e. processing is necessary for the performance of tasks carried out in the public interest – proper management of the Contracting Authority’s property, i.e. conducting a valid public procurement procedure ensuring the proper expenditure of public funds.
