1. The subject of the contract is the implementation of the sentence entitled: Modernization of parts of Mental Health Centers for the purposes of 3 rooms under the grant project entitled “Infrastructure support for Mental Health Centers for adults and centers / teams for community psychological and psychotherapeutic care (first reference level)”. 2. The investment is being implemented under grant award agreement No. U-30397/2026/00/305/2026/258 for grant application No. W-30397 under grant project No. FENX.06.01-IP.03-0007/24 entitled “Infrastructure support for Mental Health Centers for adults and centers / teams for community psychological and psychotherapeutic care (first reference level)”, implemented under the European Funds for Infrastructure, Climate and Environment Program 2021-2027, co-financed by the European Regional Development Fund, implemented on the basis of Decision on co-financing of Project No.: FENX.06.01-IP.03-0007/24-00 adopted by the Minister of Health on 28 February 2025. 3. A detailed description of the subject of the contract is contained in separate attachments – the DESCRIPTION OF THE SUBJECT OF THE CONTRACT (OPZ) file in Part B of the SWZ. The place of performance of the Contract is: Independent Public Healthcare Institution Central Clinical Hospital of the Medical University of Łódź, with its registered office in Łódź at ul. Pomorska 251, 92-213 Łódź, location: PSYCHIATRY building B-1, ul. Czechosłowacka 8/10, 92-216 Łódź 4. Designation according to the Common Procurement Vocabulary CPV: 45000000-0 Construction work; 45215140-0 Construction work for hospital facilities; 45400000-1 Building completion work; 71220000-6 Architectural design services - additional CPV in the OPZ. 5. If specific technical solutions are indicated in the description of the subject of the contract, the use of equivalent solutions is permitted with regard to their features and parameters – specified for a given item of the subject of the contract, and all possible brand names of devices and products used in the description of the subject of the contract should be treated as standard definitions, and not as specific brand names of devices and products used in this documentation. Provisions concerning equivalence are set out in point IV of the SWZ. 6. The Contracting Authority imposes the requirement referred to in Art. 95(1) of the Act, namely the employment by the Contractor or subcontractor, under an employment contract, of persons performing activities during the performance of the contract in the scope of carrying out construction work - precise information is contained in the SWZ. 7. In the case of this procedure, the Contracting Authority refrained from dividing the contract into lots because it considered that it was not possible to divide the contract into lots. The Contracting Authority did not divide the contract into lots due to the nature of the contract (continued in the SWZ). 8. The contract will be performed within a maximum period up to 31.03.2027. 9. The scope and conditions of amendments are contained in the model contract constituting an attachment to the SWZ. 10.
