(Numbering in accordance with the SWZ) 4.1. The subject of the contract is the modernization of street surfaces in the City of Łódź, divided into 4 parts. The scope of the planned works includes: surface milling, construction of new structural layers and bituminous layers, renovation of sidewalks, pedestrian and bicycle routes and stop platforms, construction of stopping bays for bus stops and accompanying infrastructure, vertical and horizontal road markings, planting trees, establishing green areas. 4.1.1.Part I task 1.1 – Modernization of the surface of Łyżwiarska Street on the section from Popiełuszki Street to Gimnastyczna Street task 1.2 – Modernization of the surface of Dubois Street on the section from Hoffmanowej Street to Polarna Street, together with Polarna Street on the section from Dubois Street to Prądzyńskiego Street task 1.3 – Modernization of the surface of Pabianicka Street – on selected sections together with the Lotników Lwowskich roundabout 4.1.2.Part II task 2.1 – Modernization of the surface of Okólna Street on the section from Łagiewnicka Street toward Serwituty Street task 2.2 – Modernization of the surface of Tkacka Street together with the link connecting Tkacka Street with Zelwerowicza Street at property No. 30A 4.1.3.Part III task 3.1 – Modernization of the surface of the CPR on Puszkina Street – on the section from Rokicińska Street to Dąbrowskiego Street task 3.2 – Modernization of the surface of Starorudzka Street on the section from Pabianicka Street to Przestrzenna Street 4.1.4.Part IV task 4.1 – Modernization of the surface of Bema Street on the section from Światowida Street to Sikorskiego Avenue task 4.2 – Modernization of the surface of Strykowska Street - the intersection with Palki Avenue and Wojska Polskiego Street together with the eastern carriageway of Palki Avenue. 4.2. The contract has been divided into parts. The Contracting Authority permits the Contractor to submit a tender for one selected part of the contract. 4.3. A detailed description of the subject of the contract is contained in Appendix No. 1 to the SWZ and in the draft contract constituting Appendix No. 7 to the SWZ (one draft for all parts of the procedure). Ignacego Skorupki Street, 90-532 Łódź tel.: +48 (42) 638-48-88 e-mail: zamowienia@uml.lodz.pl 33.1 Pursuant to Art. 95 sec. 1 of the Pzp Act, the Contracting Authority requires the Contractor or Subcontractor (for each part separately) to employ, on the basis of an employment relationship, persons performing activities specified by the Contracting Authority within the scope of contract performance, if the performance of those activities consists in performing work in the manner specified in Art. 22 §1 of the Act of 26 June 1974 – Labour Code (Journal of Laws of 2025, item 277), i.e. persons performing, during contract performance, work involving: driving or operating road equipment. 33.2 The Contracting Authority has the right to require the Contractor to present a photocopy of the current employment contract. The photocopy of the contract should contain the employee’s first name and surname, the date the contract was concluded, the type of employment contract, the working-time basis, and the scope of the employee’s duties. 33.3 The type of activities related to contract performance, the method of verifying the employment of these persons, the Contracting Authority’s powers regarding verification of the Contractor’s compliance with the requirements related to the employment of these persons, and the sanctions for failure to meet them have been specified in the description of the subject of the contract (Appendix No. 1 to the SWZ) and in the draft contract (Appendix No. 7to the SWZ. 27. INFORMATION ON THE ENVISAGED CONTRACTS REFERRED TO IN ART. 214 SEC. 1 POINTS 7 AND 8 OF THE PZP ACT, IF THE CONTRACTING AUTHORITY ENVISAGES AWARDING SUCH CONTRACTS Within a period of 3 years from the date of awarding the basic contract, the Contracting Authority envisages the possibility of awarding contracts referred to in Art. 214 sec. 1 point 7 of the Pzp Act (the possibility of awarding a contract consisting in the repetition of similar services) for all parts of the contract up to 100% of the estimated value of the basic contract on selected roads of a similar nature located within the City of Łódź and administered by the Roads and Transport Authority. Contracts within the above-mentioned scope will be awarded on the basis of negotiations with the existing Contractor, under the conditions specified in the basic contract. 24.1. The Contractor whose tender has been selected is obliged to provide performance security in the amount of 5% of the total gross price stated in the tender for the given part of the contract. 16.7. Pursuant to Art. 257 of the Pzp Act, the Contracting Authority does not envisage cancelling this procedure if the public funds that the Contracting Authority intended to allocate to finance all or part of the contract have not been granted to it. 4.1.
