Opportunity description

1. The subject of this contract is the execution, under the “design and build” formula, of design documentation and construction works consisting of the modernization of the conference room for the M. Kopernik Regional Multispecialty Center for Oncology and Traumatology in Łódź. 2. The scope of the contract includes the design and execution of construction works, together with obtaining, on behalf of the Contracting Authority, all necessary decisions, arrangements, opinions, permits and other documents (if necessary) enabling the construction and installation works to be carried out in accordance with the applicable regulations and the subsequent use of the modernized facilities of the Kopernik Hospital in Łódź. 3. All works must be carried out in accordance with the Functional and Utility Program (hereinafter referred to as the PFU), constituting Appendix No. 2 to the SWZ. 4. Under this contract, the Contracting Authority has provided for the possibility of exercising the option right. The option consists of commissioning the Contractor to perform the Optional Scope, comprising a complete mechanical ventilation system for the conference room. The detailed material, technical and functional scope of the Optional Scope is specified in the draft contract and Appendix No. 11 to the draft contract – “Description of the Optional Scope – conference room ventilation system”. 5. The description of the subject matter of the contract must be read together with any amendments to the content of the document, resulting, for example, from answers provided to contractors’ inquiries.
