Opportunity description

1. The subject of the contract is the supply of milk and milk products and substitutes to SPSK im. prof. A. Grucy CMKP in Otwock. 2. A detailed description of the subject of the contract is contained in the price form constituting Annex No. 1A and the description of the subject of the contract constituting Annex No. 5 to the SWZ. 1. As of the tender submission date, the Contractor is required to submit: a) Tender form using the template constituting Annex No. 1 to the SWZ b) Price form using the template constituting Annex No. 1A to the SWZ. c) Contractor's declaration that it meets the conditions for participation in the procedure and is not subject to exclusion (ESPD) – according to the template constituting Annex No. 2 to the SWZ. d) Declaration by the entity making resources available that it is not subject to exclusion (ESPD) – according to the template constituting Annex No. 2 to the SWZ. (if applicable) e) Declaration by Contractors jointly applying for the award of the contract – according to the template constituting Annex No. 2A to the SWZ (if applicable) f) Declaration demonstrating the absence of grounds for exclusion – according to the template constituting Annex No. 2 D to the SWZ g) Declaration demonstrating the absence of grounds for exclusion concerning the entity making resources available – according to the template constituting Annex No. 2D to the SWZ (if applicable) h) Commitment of the entity making resources available (if applicable) i) Power of attorney (if applicable) a) Subject-matter evidence referred to in Chapter VIII of the SWZ NOTE!!! Annexes No. 1 and 1A to the SWZ constitute the content of the tender and therefore may not be supplemented. The absence of the above-mentioned documents renders the submitted tender invalid. The subject-matter evidence referred to in Chapter VIII point 1 subpoints 2–6 of the SWZ may not be supplemented – in the absence of the documents, the tender will be rejected.
