Poland – Disposable non-chemical medical consumables and haematological consumables – Successive supply of disposable equipment

Opportunity description

1. The subject matter of the contract is the successive supply of disposable equipment, divided into 66 Packages. 2. The Contracting Authority allows partial tenders to be submitted, divided into 66 Packages: Packages for which tenders may be submitted for individual items – DIVISIBLE: 4,6,10,11,28,39,40,52,54,56; the remaining packages are indivisible, with no possibility of submitting tenders for individual items. A detailed description of the subject matter of the contract is set out in Annex No. 9 to the SWZ. 3. Delivery of the subject matter of the contract to the Warehouse of the Dr. Karol Jonscher Municipal Medical Centre in Łódź (ul. Milionowa 14, 93-113 Łódź) will be carried out at the Contractor’s expense and risk; in the case of Packages 49, 50, 51, 52, 53, 54, 55, 66, delivery shall be made to the Hospital Pharmacy (ul. Milionowa 14, 93-113 Łódź). For Package 65, the Contracting Authority requires monitors compatible with the offered disposable equipment to be loaned for the entire duration of the contract. The loaned equipment should have all documents required for medical equipment under the provisions of law (instructions for use in Polish, a current technical passport, a document confirming that the equipment is a medical device).
