Opportunity description
1. The subject matter of the contract is the SUPPLY OF SETS FOR THE IMPLANTATION OF INTRAVENOUS PORTS AND NEEDLES AND SYRINGES FOR PORTS for the NMP Regional Specialist Hospital in Częstochowa. 2. The description of the subject matter of the contract is contained in the assortment and price forms constituting Appendix No. 2 to the SWZ. The remaining conditions concerning the performance of the contract are specified in the draft contractual provisions (PPU), constituting Appendix No. 6 to the SWZ. 3. The subject matter of the contract is divided into 6 parts, i.e.: PART 1 – MICRO VASCULAR PORT WITH INTRODUCTION SET AND DRAPE PART 2 - LOW-PROFILE VASCULAR PORT WITH INTRODUCTION SET AND DRAPE PART 3 - STANDARD VASCULAR PORT WITH INTRODUCTION SET AND DRAPE PART 4 – SYRINGE CONTAINING 0.9% NaCL SOLUTION and CAP COMPATIBLE WITH LUER LOCK PART 5 – HUBER-TYPE NEEDLE WITH TUBING PART 6 - HUBER-TYPE NEEDLE WITHOUT TUBING 4. The Contracting Authority permits the submission of partial tenders. A partial tender means individual parts specified in Appendix No. 2 to the SWZ. The Contractor may submit a tender for any parts or for the entire subject matter of the contract. 5. CPV code: 33141000-0, 33141320-9 6. If the tender documentation indicates the origin (brand, trademark, manufacturer, supplier) of materials or equipment, or the standards, approvals, specifications and systems referred to in Art. 101 of the Pzp, the Contracting Authority permits the offering of equivalent materials, equipment or solutions, provided that they ensure technical parameters no worse than those stipulated in the above-mentioned documents. A Contractor that refers in its tender to solutions equivalent to those described by the Contracting Authority is obliged to demonstrate that the materials or equipment it offers meet the requirements specified by the Contracting Authority. 7. The Contracting Authority permits the performance of part of the contract to be entrusted to a subcontractor. The Contracting Authority does not stipulate an obligation for the Contractor to personally perform key parts of the contract. If the performance of the contract is entrusted to subcontractors, the Contractor is obliged to indicate in the Tender Form the part of the contract that it intends to entrust to subcontractors and to provide the names of any subcontractors, if already known. In the absence of such a declaration, the Contracting Authority will consider that the Contractor will perform the contract without the participation of subcontractors. 8. Electronic auction. The Contracting Authority provided in the contract notice that the selection of the most advantageous tender will be preceded by an electronic auction if at least 2 tenders not subject to rejection are submitted in this procedure. INFORMATION ON SUBJECT-MATTER EVIDENCE: 1. In order to confirm that the supplies offered comply with the requirements specified in the description of the subject matter of the contract related to the performance of the contract, the Contracting Authority requires the following subject-matter evidence to be submitted with the tender: a) descriptions of the subject matter of the contract, i.e. catalogue sheets / descriptions / folders / photographs / manufacturer’s information / leaflets / brochures / instructions for use / Contractor’s declaration* - confirming fulfilment of all parameters required by the Contracting Authority and described in Appendix 2 to the SWZ (assortment and price forms). *The Contractor’s declaration applies only where the parameters required by the Contracting Authority are not confirmed in the documents (catalogue sheets / descriptions / folders / photographs / manufacturer’s information / leaflets / brochures). Documents or declarations drawn up in a foreign language shall be submitted together with a translation into Polish; b) a subject-matter declaration confirming the compliance of the offered product classified as a medical device with the Act of 7 April 2022 on medical devices (Journal of Laws of 2024, item 1620) – in accordance with the template contained in Appendix No. 8 to the SWZ; c) samples of the offered assortment in the quantities required and described in Appendix No. 2 to the SWZ (assortment and price forms), constituting the assortment and price forms, in order to verify the necessary parameters and requirements of the offered subject matter of the contract. The samples must bear a label in Polish enabling identification of the trade name, manufacturer’s name and catalogue number (if any), EAN code (if any). 2. The Contractor shall submit the subject-matter evidence together with the tender (Art. 107(1) of the Pzp). 3. If the Contractor has not submitted the subject-matter evidence or the submitted subject-matter evidence is incomplete, the Contracting Authority shall call for its submission or supplementation within a specified time limit (Art. 107(2) of the Pzp Act). 4. The provisions of paragraph 3 shall not apply if the subject-matter evidence serves to confirm compliance with the characteristics or criteria specified in the description of the tender evaluation criteria or if, despite the submission of the subject-matter evidence, the tender is subject to rejection or grounds exist for cancelling the procedure (Art. 107(3) of the Pzp). 5. The Contracting Authority may request explanations from Contractors concerning the content of the subject-matter evidence (Art. 107(4) of the Pzp). 6. During the examination and evaluation of tenders, the Contracting Authority may request explanations from Contractors concerning the content of the submitted tenders and subject-matter evidence or other submitted documents or declarations (Art. 223(1), first sentence, of the Pzp). 7. Entity-related evidence, subject-matter evidence and other documents or declarations drawn up in a foreign language shall be submitted together with a translation into Polish (§ 5 of the Regulation of the President of the Council of Ministers of 30 December 2020 on the method of preparing and transmitting information and technical requirements for electronic documents and means of electronic communication in a public procurement or design contest procedure). REQUIREMENTS CONCERNING THE BID BOND. A Contractor participating in the procedure is obliged to provide a bid bond before the deadline for submission of tenders in the amount of PLN 5,230.00. The amount of the bid bond for the individual parts of the contract is specified in Chapter X of the SWZ. The subject matter of the contract for Part 1 is a MICRO VASCULAR PORT WITH INTRODUCTION SET AND DRAPE. The detailed scope of the contract is specified in Appendix No. 2 to the SWZ – the assortment and price form. The subject matter of the contract for Part 2 is a LOW-PROFILE VASCULAR PORT WITH INTRODUCTION SET AND DRAPE. The detailed scope of the contract is specified in Appendix No. 2 to the SWZ – the assortment and price form. The subject matter of the contract for Part 3 is a STANDARD VASCULAR PORT WITH INTRODUCTION SET AND DRAPE. The detailed scope of the contract is specified in Appendix No. 2 to the SWZ – the assortment and price form. The subject matter of the contract for Part 4 is a SYRINGE CONTAINING 0.9% NaCL SOLUTION and a CAP COMPATIBLEWITH LUER LOCK.
