Opportunity description

1. The subject of the contract is: the successive purchase of dressing materials and medical devices – together with their delivery at the Contractor’s expense and risk to the Pharmacy Warehouse of SPZOZ in Sanok, ul. 800-lecia 26, with the ordered assortment divided into individual packages, Common Procurement Vocabulary: CPV – 33.14.11.10-4 - dressings CPV – 33.14.00.00-3 - medical materials The products offered must be authorised for marketing or use in accordance with the applicable regulations. 2. A detailed description, scope and size of the subject of the contract and the contract performance conditions are contained in the price form for the subject of the contract constituting Annex No. 2 to the SWZ and in the model contract, Annex No. 4 to the SWZ, constituting an integral part of the SWZ. 3. The quantities specified in the price form are estimated/forecast values which, during the term of the contract, may be reduced depending on the current needs of the Contracting Authority. During the term of the contract, the Contracting Authority guarantees orders at a minimum level of 25% of the value of the concluded contract. 4. The Contractor shall offer a 60-day payment period from the date of issuance of a correct VAT invoice and shall offer a period of unchanged prices for the term of the contract. 5. If the Contractor finds that the content of the SWZ (including the annexes), within the scope concerning the description of the subject of the contract, refers to a trademark, patent or origin, source or particular process characterising a specific product, it shall be assumed that such reference is accompanied by the words: “or equivalent”. The Contracting Authority shall consider as an equivalent assortment one that has the same or better technical and quality parameters than those described in the SWZ and whose use will not negatively affect the proper functioning/use of the assortment in accordance with its intended purpose. A Contractor that relies on solutions equivalent to those described by the Contracting Authority is obliged to demonstrate that the supplies it offers meet the requirements specified by the Contracting Authority and will not result in additional costs arising from their use. In each case where standards, technical assessments, technical specifications and technical reference systems referred to in Article 101(1)(2) and Article 101(3) of the Pzp Act are used in the
