Opportunity description

I. 1. The subject matter of the contract is environmental supervision in the field of active bat protection in the field of active bat protection, necessary for the proper performance of the works carried out as part of the investment, i.e. without harm to bats. 2. The task of the supervision will be close cooperation with the Contracting Authority and the lighting modernization Contractor in determining details of the planned works that are important for bats and continuously monitoring their progress. Numerous site visits will be necessary. After completion of the works, the task of the supervision will be to issue an opinion on and accept them. 3. The church building is a monument entered in the register of monuments together with its immediate surroundings within the fence. 2. The Contracting Authority – pursuant to Article 95(1) of the Act – requires the Contractor or subcontractor to employ, on the basis of an employment relationship, persons assigned by the Contractor or subcontractor to perform activities consisting in carrying out work in the manner specified in Article 22 § 1 of the Act of 26 June 1974, the Labour Code. 3. Details of the scope of the subject matter of this contract are specified in Appendix No. 9 to the SWZ – Description of the Subject Matter of the Contract (OPZ). II. The tender in the procedure shall be submitted, under pain of invalidity, in electronic form, i.e. in electronic form bearing a qualified electronic signature. III. 1. Documents submitted by the contractor together with the tender: 1) completed Tender Form (Appendix No. 1 to the SWZ); if the contractor wishes to receive additional points under the second tender evaluation criterion, it shall submit a list of the expert’s additional experience 2) a power of attorney or another document confirming authorization to represent the contractor – if the tender or the statements or documents attached thereto are signed by an attorney-in-fact. 3) a power of attorney to represent all Contractors jointly applying for the award of the contract, or alternatively a cooperation agreement from which the power of attorney in question will arise. 4) JEDZ (in accordance with the instructions in Appendix No. 2 to the SWZ), valid as at the tender submission date, for the purpose of provisionally confirming that the Contractor is not subject to exclusion from the procedure pursuant to Article 108(1) and Article 109(1)(4) of the Pzp Act and meets the conditions for participation in the procedure specified in the SWZ. Where Contractors jointly apply for the contract, a JEDZ shall be submitted by each of the Contractors jointly applying for the contract; 5) a JEDZ concerning other entities on whose resources the Contractor relies, for the purpose of provisionally confirming, as appropriate as at the tender submission date, the absence of grounds for their exclusion pursuant to Article 108(1) and Article 109(1)(4) of the Pzp Act and their fulfilment of the conditions for participation in the procedure specified in the SWZ, to the extent to which the Contractor relies on their resources; 6) a statement by the Contractor, valid as at the tender submission date, for the purpose of confirming that the Contractor is not subject to exclusion from the procedure pursuant to Article 5k of Council Regulation (EU) 2025/2033 and Article 7(1) of the Act on special solutions for counteracting support for aggression against Ukraine and protecting national security, in accordance with the template constituting Appendix No. 8 to the SWZ. Where Contractors jointly apply for the contract, the statement shall be submitted by each of the Contractors jointly applying for the contract; 7) a statement by another entity on whose resources the Contractor relies, submitted for the purpose of confirming, as at the tender submission date, that this entity is not subject to exclusion from the procedure pursuant to Article 5k of Council Regulation (EU) Regulation 2025/2033 and Article 7(1) of the Act on special solutions for counteracting support for aggression against Ukraine and protecting national security, in accordance with the template constituting Appendix No. 8 to the SWZ; 8) where the Contractor relies on the capacities or situation of entities making resources available: evidence confirming that, when performing the contract, the Contractor will have at its disposal the necessary resources of other entities, which shall consist in particular of an undertaking by those entities to place the necessary resources at the Contractor’s disposal for the purposes of performing the contract, or another entity-related means of evidence confirming that, when performing the contract, the contractor will have at its disposal the necessary resources of those entities. 9) a statement by Contractors jointly applying for the award of the contract indicating which supplies or services will be performed by the individual contractors, in accordance with the template constituting Appendix No. 7 to the SWZ (if applicable); V.
