1. The subject of the contract is the supply and installation of heat pumps together with the necessary infrastructure for the Social Welfare Home in Chlebno. 1.1. The contract includes the performance of installation, hydraulic and electrical works, as well as the integration of the system with the existing photovoltaic and central heating installations, including: 1) purchase and installation of a 17.4kW air-to-water heat pump – 3 pcs., 2) purchase and installation of cascade control automation – 1 pc., 3) purchase and installation of a domestic hot water tank – 3 pcs., 4) purchase and installation of a buffer tank – 1 pc., 5) purchase and installation of an immersion heater – 1 pc., 6) purchase and installation of a heater for the domestic hot water tank – 3 pcs., 7) purchase and installation of a mounting bracket – 3 pcs., 8) purchase and installation of a brass three-way valve – 3 pcs., 9) purchase and installation of a valve actuator – 3 pcs., 10) purchase and installation of an electronic circulation pump – 3 pcs., 11) purchase and installation of a temperature sensor – 5 pcs. 1.2. The contract includes the integration of the system with the existing central heating installation – the oil-fired boiler room. The Contractor shall perform all necessary preparatory works (e.g. construction of foundations). 1.3. A detailed description of the subject of the contract is contained in Annex No. 3 to the SWZ. 2. THE PROJECT ENTITLED “Purchase and installation of a heat pump in the building of the Social Welfare Home in Chlebno” WAS CO-FINANCED FROM THE FUNDS OF THE PROVINCIAL FUND FOR ENVIRONMENTAL PROTECTION AND WATER MANAGEMENT IN POZNAŃ IN THE FORM OF A GRANT IN THE AMOUNT OF PLN 300,000.00. 3. The Contracting Authority requires the Contractor to conduct a site visit: 1) submission of a tender must be preceded by a site visit, 2) site visit location: Chlebno 8, 89-310 Łobżenica, 3) a site visit is possible by prior appointment, on the following days: Monday to Friday, from 9:00 to 13:00, 4) to arrange a site visit, please contact Ms Paulina Paczkowska, tel. 67 286 00 14 or by e-mail: dps@dpswchlebnie.pl. Information confirming completion of the site visit will be drawn up following the site visit – the template constitutes Annex No. 8 to the SWZ, 5) submission of a tender without completing a site visit will result in rejection of the tender. 4. In order to confirm that the offered supply complies with the requirements specified by the Contracting Authority in Annex 3 to the SWZ, the Contracting Authority requires the following subject-matter evidence to be submitted with the tender: a document confirming compliance with the heat pump parameters specified in the technical specification, e.g. a technical data sheet, energy label, certificates or a declaration from the pump manufacturer. If the Contractor has not submitted the subject-matter evidence or the submitted subject-matter evidence is incomplete, the Contracting Authority will request its submission or supplementation within a specified time limit. 5. Warranty period and statutory warranty for defects for the subject of the contract: min. 5 years. The warranty and statutory warranty period for the subject of the contract constitutes a tender evaluation criterion in accordance with point 16 of the SWZ. 6. The requirements concerning the subject of the contract specified in the SWZ and Annex No. 3 to the SWZ – Description of the subject of the contract, are minimum requirements. The Contracting Authority permits the offering of the subject of the contract with better parameters. 7. The supplied equipment must be factory-new, unused and not displayed at exhibitions or trade fairs, and must meet the Contracting Authority's requirements specified in Annex No. 3 to the SWZ. The subject of the contract must be technically operational, complete and ready for operation. The supplied equipment must be accompanied by appropriate documents/certificates confirming compliance with all applicable standards and guidelines that the aforementioned subject of the contract should meet before being approved for use, as well as warranty cards and operating instructions in Polish – if the equipment has them. 8. If the documents describing the subject of the contract contain manufacturers' names, they serve to define the desired standard of contract performance. The Contractor may offer an equivalent subject of the contract corresponding at least to the parameters of the specified equipment. 9. Pursuant to Art. 101(4) of the Pzp Act, the Contracting Authority permits equivalent standards, technical assessments, technical specifications and technical reference systems. 10. The Contractor undertakes to bear the costs of transport, unloading and insurance until acceptance. 11. The Contractor assumes full liability for any violations of law and damages that may arise in connection with the performance of the contract. 12. The Contractor should have a sanitary works manager at its disposal. The selected Contractor shall provide the manager's details to the Contracting Authority before conclusion of the contract. 14.
