1. The subject matter of the contract is: Comprehensive insurance for the Independent Public Healthcare Institution in Przeworsk” 2. Type of contract: service. 3. The subject matter of the contract is classified in the Common Procurement Vocabulary under CPV codes: 66510000-8 - insurance services, 66515000-3 - damage or loss insurance services, 66516000-0 - liability insurance services, 66516100-1 - motor vehicle liability insurance services, 66514110-0 - motor vehicle insurance services, 66512100-3 accident insurance services. 4. The subject matter of the contract has been divided into lots. The insurance includes: Lot I: Property insurance a) All-risk property insurance Lot II: Liability insurance a) Compulsory liability insurance for an entity carrying out medical activities, b) Voluntary liability insurance for damage caused to a third party as a consequence of providing healthcare services or the unlawful failure to provide healthcare services in connection with carrying out medical activities, c) Voluntary liability insurance for damage caused to a third party arising from the activities conducted and property owned. Lot III: Motor insurance a) Compulsory third-party liability insurance for owners of motor vehicles (OC ppm.), b) Comprehensive motor insurance (AC+ASS), c) Personal accident insurance for the driver and passengers (NNW), 5. The Contracting Authority does not provide for the possibility of applying the option right. 7.
