Opportunity description

The subject of the contract is the successive supply by the Contractor to the Contracting Authority of disposable items together with the lease of apparatus for the Microbiology Laboratory and the Pathomorphology Department (hereinafter referred to as the goods), described in detail and in the quantities specified in the Price Form constituting Appendix No. 2 to the SWZ, together with the lease of the apparatus (hereinafter also referred to interchangeably as the device or the subject of the lease), described in detail in the List of Required Functions and Technical Parameters constituting Appendix No. 6 to the SWZ, and in accordance with the draft contractual provisions constituting Appendix No. 4 to this SWZ.- (this provision applies to the task involving a lease – task No. 1). The Contracting Authority guarantees that the minimum value of contracts awarded to the Contractor within a given task throughout the entire term of the contract will amount to 70% of the maximum value of the Contractor’s remuneration for that task, as specified in the price form constituting Appendix No. 1 to the contract. The Contracting Authority informs that, in the present procedure conducted as an open tender, it provides for the possibility of applying the procedure referred to in Article 139(1) of the p.z.p. Act. When applying the aforementioned procedure, the Contracting Authority will first examine and evaluate the tenders and will then conduct the entity qualification of the Contractor whose tender received the highest evaluation with regard to the absence of grounds for exclusion and fulfilment of the conditions for participation in the procedure. The subject of the contract is the successive supply by the Contractor to the Contracting Authority of containers preloaded with slides (hereinafter referred to as the goods), described in detail and in the quantities specified in the Price Form constituting Appendix No. 2 to the SWZ, and in accordance with the draft contractual provisions constituting Appendix No. 4 to this SWZ.
