The subject matter of the contract is the successive supply by the Contractor to the Contracting Authority of disposable products together with the lease of equipment for the Blood Bank, Microbiology Laboratory, Analytical Laboratory and Pathomorphology Department (hereinafter referred to as the goods), described in detail and in the quantities specified in the Pricing Form constituting Appendix No. 2 to the SWZ, together with the lease of equipment (hereinafter also referred to interchangeably as the device or the subject of the lease), described in detail in the List of Required Functions and Technical Parameters constituting Appendix No. 6 to the SWZ, and in accordance with the draft contractual provisions constituting Appendix No. 4 to this SWZ.- (this provision applies to tasks involving a lease – tasks Nos. 1, 2). The Contracting Authority guarantees that the minimum value of supplies during the term of the contract will amount to: • 70% of the maximum value of the Contractor's remuneration under this task, specified in the pricing form constituting Appendix No. 2 to the SWZ.- does not apply to tasks Nos. 6, 11. • 50% of the maximum value of the Contractor's remuneration under this task, specified in the pricing form constituting Appendix No. 2 to the SWZ.- applies to tasks Nos. 6, 11. The Contracting Authority hereby states that, in this procedure conducted as an open tender, it provides for the possibility of applying the procedure referred to in Article 139(1) of the Public Procurement Law. When applying the aforementioned procedure, the Contracting Authority will first examine and evaluate the tenders and will then carry out the qualification of the Contractor whose tender was ranked highest with regard to the absence of grounds for exclusion and fulfilment of the conditions for participation in the procedure. The subject matter of the contract is the successive supply by the Contractor to the Contracting Authority of RT PCR tests dedicated to pathogen detection, extraction kits and packages for collecting clinical specimens (hereinafter referred to as the goods), described in detail and in the quantities specified in the Pricing Form constituting Appendix No. 2 to the SWZ, together with the lease of equipment (hereinafter also referred to interchangeably as the device or the subject of the lease), described in detail in the List of Required Functions and Technical Parameters constituting Appendix No. 6 to the SWZ, and in accordance with the draft contractual provisions constituting Appendix No. 4 to this SWZ. The subject matter of the contract is the successive supply by the Contractor to the Contracting Authority of microbiological media on disposable plates (90 mm), in test tubes and bottles; (hereinafter referred to as the goods), described in detail and in the quantities specified in the Pricing Form constituting Appendix No. 2 to the SWZ, and in accordance with the draft contractual provisions constituting Appendix No. 4 to this SWZ.
