Opportunity description
1) The subject matter of the contract is group life insurance for employees. 2) A detailed description of the subject matter of the contract is contained in Part II of the SWZ and Part IV of the SWZ. 3) Pursuant to the provisions of 438 of the Act in conjunction with Art. 95(1) of the Act, the requirements concerning the obligation to employ employees are set out in Part II of the SWZ and Part IV of the SWZ. 4) The Contracting Authority requires the Contractor to indicate, in the tender, the parts of the contract which it intends to entrust to Subcontractors and to provide the names of the Subcontractors, if already known. 5) The Contracting Authority does not reserve the obligation for the Contractor to personally perform key tasks relating to the services. 6) Name and code of the Common Procurement Vocabulary: Code: 66511000-5 Life insurance services. 7) The Contracting Authority does not require the submission of subject-matter evidence with the tender. 8) Acting pursuant to Art. 133(3) of the Act, the Contracting Authority establishes requirements concerning the preservation of the confidential nature of information provided to Contractors during the procedure. The following parts of the SWZ are confidential: Part II of the SWZ – Description of the subject matter of the contract. Part IV of the SWZ – Draft contractual provisions. 9) In accordance with Art. 133(3) of the Act, the Contracting Authority does not make the content listed in point 8 publicly available on the website due to the protection of the confidential nature of the information contained in the SWZ. The files posted on the procurement platform named “Part II of the SWZ – Description of the subject matter of the contract” and “Part IV of the SWZ – Draft contractual provisions” have been password-protected. In order to protect the confidential nature of the Confidential Information made available during this procedure, the Contracting Authority stipulates that the password for the confidential information referred to above will be provided after the Contractor submits a confidentiality declaration bearing a qualified electronic signature. The declaration must be submitted by persons authorized to represent the Contractor (this authorization must arise from registration documents or a power of attorney attached to the submitted declaration) and by persons who will participate in the procedure. Templates of the confidentiality declarations constitute Annexes Nos. 3.1 and 3.2 to Part III of the SWZ. The Contracting Authority informs that letters containing clarifications of the content of the SWZ concerning the classified part of the contract (if any are submitted during the procedure) will also be published in encrypted form—the above-mentioned password must be used for decryption (this password will be used throughout the entire procedure). The declarations must be submitted via the Procurement Platform, under the “Questions/Information” tab. After they have been received and verified, the password for decrypting the documents will be sent to the email address indicated in the information requesting the password for decrypting the documents. Note: If the persons who will participate in the procedure do not have qualified electronic signatures, the Contracting Authority permits these declarations to be signed by hand by the persons making the declarations, scanned, certified as true copies of the originals by persons authorized to submit declarations of intent and knowledge on behalf of the Contractor, and submitted in this form to the Contracting Authority via the Procurement Platform.
