Opportunity description

The subject matter of the procedure is: Group life insurance service for employees and family members of employees of the City of Chorzów together with its organizational units. The Contracting Authority does not guarantee that all employed staff and family members of the Contracting Authority's employees will enroll in the group life insurance. During the insurance period, changes may occur in the number of insured persons. If an employee or an employee's family member withdraws from the insurance, the Contracting Authority is not obliged to pay the insurance premium on their behalf. The detailed terms and conditions of the contract, together with the description of the subject matter of the contract and the age and gender structure, are provided in the annexes to the SWZ. The Contractor must have general (special) insurance terms and conditions (hereinafter referred to as the OWU) for all insurance specified in the subject matter of the contract. Before concluding the public procurement contract, the selected Contractor is obliged to provide the Contracting Authority, through the Contracting Authority's representative, Supra Brokers S.A., with a standardized document containing information on the insurance product referred to in Article 8(4) of the Act of 15 December 2017 on Insurance Distribution, for the individual insurance policies constituting the subject matter of the contract, together with the OWU.
