Opportunity description

The subject matter of the contract is the supply of instruments and products for the DA VINCI X surgical robot owned by the Contracting Authority, knee joint prostheses, hip joint endoprostheses, including a revision endoprosthesis, bone fixation implants, cemented and uncemented hip joint endoprostheses, shoulder endoprostheses, implants and accessories for soft tissue repair – knee, a dual-mobility carpometacarpal joint endoprosthesis, bone substitute, a membrane for osteochondral reconstruction in the knee and ankle joints, wrist endoprostheses, products for orthopedic procedures – cruciate ligament reconstruction, components and implants necessary for soft tissue repair (shoulder), and scaffolds for articular cartilage reconstruction, divided into 14 lots. Pursuant to Art. 139 of the PPL, the Contracting Authority informs that it will first examine and evaluate the tenders in this public procurement procedure and will then assess the eligibility of the contractor whose tender received the highest evaluation with regard to the absence of grounds for exclusion and fulfillment of the conditions for participation in the procedure. DOCUMENTS SUBMITTED WITH THE TENDER 1. The tender consists of: 1) a completed tender form, in accordance with Appendix No. 1 to the SWZ; 2) a completed product and price form, in accordance with Appendix No. 2 to the SWZ. 2. Together with the tender, the Contractor is required to submit: 1) an extract or information from the National Court Register, the Central Register and Information on Economic Activity, or another appropriate register – in order to confirm that the person acting on behalf of the Contractor is authorized to represent it. The Contractor is not required to submit these documents if the Contracting Authority can obtain them using free and publicly accessible databases, provided that the Contractor has indicated the data enabling access to these documents; 2) if a person whose authority to represent the contractor does not arise from the documents referred to in the preceding point acts on behalf of the contractor: a power of attorney or another document confirming authorization to represent the contractor; 3) a power of attorney to represent Contractors jointly applying for the award of the contract – if applicable; 4) where the bid bond is provided in a form other than money – the original bid bond document (guarantee or surety) – if applicable. 5) The declaration and documents indicated in Chapter VI A, to the extent specified therein as being submitted with the tender. The procedure is conducted in accordance with the Act of 11 September 2019, PPL (Journal of Laws of 2026, item 793) – hereinafter PPL, under an open tender procedure with an estimated value exceeding the EU thresholds (Art. 132 PPL), pursuant to Art. 138(4) of the PPL Act.
