1. The subject matter of the contract is the successive supply of medical devices upon the Contracting Authority’s orders. A detailed description of the subject matter of the contract is provided in Annex No. 2 to the SWZ – Product and Price Form. The subject matter of the contract has been divided into 20 individual Tasks: Task 1. Intravenous cannulas Task 2. Various medical devices 1 Task 3. Various medical devices 2 Task 4. Various medical devices 3 Task 5. Various medical devices 4 Task 6. General disposable supplies Task 7. Set for the Compact Ella pump Task 8. Laryngological accessories Task 9. ECG electrodes and accessories Task 10. Anti-thrombosis stockings Task 11. Serology supplies Task 12. Container for used needles and medical waste Task 13. Anaesthesiology supplies Task 14. Disposable equipment for the PulsioFlex device owned by the Hospital and traps for the Carescape B-450 Monitor Task 15. Anaesthesiology accessories Task 16. Blood pressure measurement set Task 17. Accessories for Philips equipment Task 18. Biliary drainage set Task 19. Disposable laryngoscopes Task 20. The medical supplies constituting the subject matter of the contract must be authorised for marketing and use, meet the requirements set out in Regulation (EU) 2017/745 of the European Parliament and of the Council of 5 April 2017 on medical devices (MDR) and in the Act of 7 April 2022 on medical devices (consolidated text: Journal of Laws of 2024, item 1620), and in particular bear the CE marking and have the appropriate declarations of conformity. 3. The Contractor is required to possess and, at the Contracting Authority’s request, submit within 3 working days all documents confirming that the offered supplies are authorised for marketing and use in accordance with applicable law. The Contractor shall ensure that these documents remain valid throughout the entire contract performance period.
