1. The subject matter of the contract is the successive supply, upon the Contracting Authority’s order, of medical devices used for procedures in the Operating Block 2. A detailed description of the subject matter of the contract, including the assortment, doses, forms and quantities, is set out in Appendix No. 2 to the SWZ – Assortment and Price Form. 3. The medical assortment constituting the subject matter of the contract must be authorized for marketing and use, meet the requirements set out in Regulation (EU) 2017/745 of the European Parliament and of the Council of 5 April 2017 on medical devices (MDR) and in the Act of 7 April 2022 on medical devices (consolidated text: Journal of Laws 2024, item 1620), in particular bear the CE marking and have the appropriate declarations of conformity.
