1. The subject matter of the contract in this tender procedure is: Supply of medical equipment necessary for the provision of cardiological care in organizational units within the structure of the University Clinical Hospital No. 2 of the Medical University of Łódź (hereinafter referred to as the Equipment), in accordance with the type of assortment and quantities specified in the “Assortment and Price Form” constituting Annex No. 2 to the SWZ, in accordance with the “Tender Form”, constituting Annex No. 1 to the SWZ, and meeting the threshold parameters specified in the annex “Technical Parameters” - Annex 1a to the Tender Form, and in accordance with the annex “Warranty and Service Conditions” - Annex 1b to the Tender Form. 2. The contract is divided into 12 parts (packages). 3. The subject matter of the contract includes the supply of: 3.1. Package No. 1 - Dental units, 3.2. Package No. 2 – AngioOCT, 3.3. Package No. 3 – Wide-angle camera for fundus examination, 3.4. Package No. 4 – Computerized perimeter, 3.5. Package No. 5 – Ophthalmic ultrasound unit, 3.6. Package No. 6 – Autokeratorefractometer, 3.7. Package No. 7 – Phoropter, 3.8. Package No. 8 – Lensmeter (dioptometer), 3.9. Package No. 9 – EMG, 3.10. Package No. 10 – EEG, 3.11. Package No. 11 – Video gastroscope, 3.12. Package No. 12 – Video colonoscope. 1.
