The subject of the contract is the supply of medical equipment and apparatus for the needs of the 105th Kresowy Military Hospital with Outpatient Clinic, Independent Public Healthcare Institution in Żary, under the PMT grant for the 105th Kresowy Military Hospital with Outpatient Clinic, Independent Public Healthcare Institution in Żary, divided into 2 lots, i.e.: Lot No. 1: Supply of an ultrasound machine Lot No. 2: Supply of an ultrasound machine for cataract examination The subject of the contract is the supply of an ultrasound machine covered by Lot No. 1 – a detailed description of the subject of the contract is provided in Annex No. 8.1 to the SWZ – the assortment and price form, and in Annex No. 9.1 to the SWZ – the required parameters form, under the terms specified in the draft contractual provisions constituting Annex No. 10 to the SWZ. The subject of the contract is the supply of an ultrasound machine for cataract examination covered by Lot No. 2 – a detailed description of the subject of the contract is provided in Annex No. 8.2 to the SWZ – the assortment and price form, and in Annex No. 9.2 to the SWZ – the required parameters form, under the terms specified in the draft contractual provisions constituting Annex No. 10 to the SWZ.
