Opportunity description

The subject of the contract is the purchase and delivery of medical apparatus and equipment for specialist outpatient clinics, the Gynecology Department, OAiIT, SOR in accordance with the description of the subject of the contract constituting Annex No. 8 to the SWZ – Description of the subject of the contract and Annex No. 7 to the SWZ – Assortment and price form.. The subject of the contract consists of 5 packages: Package No. 1 – Device for transdermal application of oxygen with a hyaluronic acid administration function Package No. 2 – Gynecological cystoscope Package No. 3 – Surgical instruments Package No. 4 – Diathermy units Package No. 5 – Videolaryngoscopes Package No. 1 – Device for transdermal application of oxygen with a hyaluronic acid administration function The subject of the contract includes in particular: a) delivery of the subject of the contract together with unloading, internal transport, installation and commissioning (if applicable) at the Contracting Authority's premises, b)delivery together with the subject of the contract of: - a technical passport and operating instructions in Polish, - the necessary equipment specified in the annexes describing the subject of the contract in detail, c) provision of at least a 24-month warranty for the subject of the delivery (calculated from the date of installation, commissioning and handover of the device, as confirmed by a protocol, within a timeframe taking into account the Contracting Authority's working hours), d) ensuring a service response within no more than 24 from the moment of notification by the Contracting Authority (in accordance with the OPZ), e) provision (included in the tender price) of training for medical personnel in the operation of the offered subject of the contract in accordance with Annex No. 8 to the SWZ - Description of the Subject of the Contract. Package No. 2 – Gynecological cystoscope The subject of the contract includes in particular: a) delivery of the subject of the contract together with unloading, internal transport, installation and commissioning (if applicable) at the Contracting Authority's premises, b)delivery together with the subject of the contract of: - a technical passport and operating instructions in Polish, - the necessary equipment specified in the annexes describing the subject of the contract in detail, c) provision of at least a 24-month warranty for the subject of the delivery (calculated from the date of installation, commissioning and handover of the device, as confirmed by a protocol, within a timeframe taking into account the Contracting Authority's working hours), d) ensuring a service response within no more than 24 from the moment of notification by the Contracting Authority (in accordance with the OPZ), e) provision (included in the tender price) of training for medical personnel in the operation of the offered subject of the contract in accordance with Annex No. 8 to the SWZ - Description of the Subject of the Contract.
