Opportunity description

1. The subject matter of the contract is the successive supply of medical gases (hereinafter interchangeably referred to as gases or goods), the type and quantity of which have been specified in the price form constituting Appendix No. 2 to the SWZ, and the successive supply of accessories for anaesthetic gas (50% oxygen / 50% nitrous oxide), i.e. mouthpieces – applies to Task No. 3 (hereinafter interchangeably referred to as goods), in the quantity specified in the price form constituting Appendix No. 2 to the SWZ, together with the lease of:  a stationary tank with vaporisers for liquid oxygen (applies to Task No. 1),  a liquid nitrogen tank and a liquid air tank for cryotherapy (applies to Task No. 2),  cylinders (applies to Tasks Nos. 2, 3, 4, 5, 6),  and accessories, i.e. cylinder trolleys and dosing valves (applies to Task No. 3) (hereinafter interchangeably referred to as the subject matter of the lease), described in detail in the price form constituting Appendix No. 1 to the agreement, as well as the remaining obligations arising from the agreement aimed at enabling the Contracting Authority to use the goods and the subject matter of the lease, and in accordance with the draft provisions of the agreement constituting Appendix No. 4 to this SWZ. 2. Detailed description of the subject matter of the contract by tasks: Task No. 1: Supplies of medical liquid oxygen together with the lease of a tank Task No. 2: Supplies of gases for cryotherapy together with the lease of tanks and cylinders Task No. 3: Supplies of 50/50% O2/N2O medical gas for anaesthesia together with the lease of cylinders, trolleys and dosing valves Task No. 4: Supplies of special gases together with the lease of cylinders Task No. 5: Supplies of medical gases together with the lease of cylinders Task No. 6: Supplies of gases for medical purposes together with the lease of cylinders in accordance with the detailed description of the subject matter of the contract contained in the Price Form constituting Appendix No. 2 to the SWZ. The Contractor may submit a tender for one, several or all parts. During the examination and evaluation of tenders, the Contracting Authority will consider each task separately. 3. In Appendix No. 2 to the SWZ, the Contracting Authority specified quality requirements relating at least to the main elements comprising the subject matter of the contract. 4. The Contractor should offer and price all assortment items listed in the price form. The Contractor is obliged to provide the manufacturer’s name or trade name in Appendix No. 2 to the SWZ. The Contracting Authority informs that, in this procedure conducted under the open tender procedure, it provides for the possibility of applying the procedure referred to in Art. 139(1) of the Public Procurement Law. When applying the aforementioned procedure, the Contracting Authority will first examine and evaluate the tenders and then perform the personal qualification of the Contractor whose tender received the highest evaluation with regard to the absence of grounds for exclusion and fulfilment of the conditions for participation in the procedure. 1.
