Opportunity description
1) The subject of the contract is the supply of single-use kits for cataract surgery, vitrectomy, intravitreal injections and intraocular lenses for the Multispecialty Hospital in Inowrocław, for a period of 24 months from the date of signing the contract. 2) The contract performance period specified as 24 months may be extended by the Contracting Authority if the Contracting Authority has not used the quantity of the assortment specified as the subject of the contract. 3) Delivery time for the current order – minimum 24 hours, maximum 72 hours. 4) Place of contract performance: Szpital Wielospecjalistyczny im. dr. Ludwika Błażka, ul. Required validity period of the subject of the contract – min. 12 months. b. Required statutory warranty period for the completed subject of the contract – 24 months. 9) The products offered must have the required certificates authorising their use in the public health service. 10) Deliveries of lenses and sets of materials for cataract surgery will be made each time after the Buyer has first placed an order by telephone and confirmed it in writing, specifying the type and quantity of the lenses and sets of materials for cataract surgery ordered 11) For the performance of the contract, the Seller will entrust the Buyer with intraocular lenses on consignment in the quantity specified in Annex No. 2A to the SWZ, i.e.: Aspheric lens with UV filter – 200 units Aspheric lens with UV and blue-light filter – 49 units multi-piece lens with UV filter – 43 units spherical lens with UV filter – 12 units aphakic anterior chamber lens fixed to the iris 12) The Seller undertakes to lend 10 lens implantation kits (forceps and injector) free of charge for the duration of the contract (24 months). 13) In Task No. 4, a lens size that was not included in the consignment stock will be purchased under a current order 14) The quantities stated in Annex No. 2 constitute estimated annual consumption and may be reduced by up to 20% of the unused quantity, without constituting an obligation on the part of the Contracting Authority to use them in full or grounds for the Contractor to pursue claims for damages due to failure to use the maximum contract value. 15) The Contracting Authority does not allow the submission of variant tenders. 16) The Contracting Authority does not require the contractor to personally perform work related to the subject of the contract.
