1. The subject matter of the contract is the successive supply of disposable items for the Department of General and Oncological Urology (hereinafter referred to as the goods), of the type and in the quantities specified in the price form constituting Appendix No. 2 to the SWZ and in accordance with the draft contractual provisions constituting Appendix No. 4 to this SWZ, i.e. by tasks as follows: Task No. 1: Supplies of sets for percutaneous nephrostomy using the two-stage two step method, Task No. 2: Supplies of sets for percutaneous nephrostomy using the single-stage method Task No. 3: Supplies of urological ureteral access sheaths; Task No. 4: Supplies of urological tubing for irrigation pumps compatible with the PV-5201U irrigation pump manufactured by Vimex Endoscopy, owned by the Contracting Authority, Task No. 5: Supplies of disposable sets for ureterorenoscopy; Task No. 6: Supplies of testicular prostheses, Task No. 7: Supplies of disposable items in accordance with the detailed description of the subject matter of the contract contained in the Price Form constituting Appendix No. 2 to the SWZ. The Contractor may submit a tender for one, several or all parts. During the examination and evaluation of tenders, the Contracting Authority will consider each task separately. The Contracting Authority informs that, in the present procedure conducted under the open tender procedure, it provides for the possibility of applying the procedure referred to in Article 139(1) of the p.z.p. Act. When applying the aforementioned procedure, the Contracting Authority will first examine and evaluate the tenders and then perform the qualitative selection of the Contractor whose tender received the highest evaluation with regard to the absence of grounds for exclusion and fulfilment of the conditions for participation in the procedure. 1.
