The subject of the contract is the provision of healthcare for employees of Poczta Polska S.A. whose personnel matters are handled by the Regional Division of the Human Resources and Payroll Office in Wrocław, in the city of Wałbrzych and the surrounding area. The contractor will be required to provide preventive healthcare for employees of Poczta Polska S.A., following a request for preventive examinations, in particular in the following scope: 1) conducting preventive examinations of employees: pre-employment, periodic and follow-up examinations provided for in the Labour Code, in accordance with the methodological guidelines for conducting preventive examinations of employees specified in the Regulation of 30 May 1996 (Journal of Laws of 2023, item 607, as subsequently amended), as well as other examinations additionally ordered by the physician conducting the examination, in accordance with the applicable regulations, 2) conducting psychological examinations for the operation of motor-powered industrial trucks used in internal transport (forklift trucks), 3) conducting medical and psychological examinations of persons applying for entry or entered on the list of qualified physical security personnel, 4) conducting medical and psychological examinations of drivers operating vehicles up to 3.5 tonnes and over 3.5 tonnes; 5) conducting medical examinations for sanitary and epidemiological purposes; 6) issuing medical certificates confirming the absence of health-related contraindications to work in a specific position or health-related contraindications to work in a specific position, in accordance with the applicable templates, 7) monitoring employees’ health, in particular to identify possible occupational diseases, as well as the causes of accidents at work, 8) initiating activities for the protection and promotion of employees’ health and providing assistance in their implementation, 9) maintaining and storing medical documentation relating to preventive healthcare in accordance with the applicable regulations, 10) participation of an occupational medicine physician in the work of workplace occupational health and safety committees. During the term of the contract, the subject of the contract will cover approximately 834 services/examinations for employees of Poczta Polska S.A. and candidates for employment at PP S.A.
