The subject matter of the contract is the provision to the Contracting Authority of occupational medicine services for employees of PP S.A. working in Zielona Góra County. The subject matter of the contract is the provision of occupational medicine services, in particular preventive, pre-employment, periodic and follow-up examinations for employees and candidates for employment. 1. The subject matter of the contract is the provision to the Contracting Authority of occupational medicine services for employees of PP S.A. working in Zielona Góra County. During the term of the contract, approximately 1139 employees will be covered by the subject matter of the contract. 2. Detailed characteristics of the positions and the numbers of employees expected to undergo examinations are contained in the Pricing Form constituting Appendix No. 2 to the SIWZ. 3. Details concerning the conditions for performance of the service are included in the Draft Agreement constituting Appendix No. 3 to the SWZ. 4. The subject matter of the contract is the provision of preventive healthcare for employees of Poczta Polska S.A. whose human resources services are handled by the Wrocław Regional Human Resources and Payroll Division, Human Resources and Payroll Services Department in Gorzów Wlkp., and other human resources units of Poczta Polska S.A. that request preventive examinations. The Contractor undertakes to provide preventive healthcare for employees of Poczta Polska S.A. that requests preventive examinations in accordance with the provisions laid down in: • the Act of 26 June 1974, the Labour Code, • the Regulation of the Minister of Health and Social Welfare of 30 May 1996 on conducting medical examinations within the scope of preventive healthcare for employees and medical certificates issued for the purposes provided for in the Labour Code, • the Act of 27 June 1997 on occupational medicine services, • the Act of 6 September 2001 on road transport concerning drivers of vehicles, • the Act of 5 January 2011 on drivers of vehicles, • the Regulation of the Minister of Health of 29 August 2019 on medical examinations of persons applying for authorisation to drive vehicles and of drivers, • the Regulation of the Minister of Health of 08 July 2014 on psychological examinations of persons applying for authorisation to drive vehicles, drivers and persons working as drivers, • the Regulation of the Minister of Development and Finance of 15 December 2017 on occupational health and safety when using powered industrial trucks, • the Act of 27 August 1997 on vocational and social rehabilitation and the employment of persons with disabilities, in particular through: 1) conducting preventive examinations of employees: pre-employment, periodic and follow-up examinations provided for in the Labour Code, in accordance with the methodological guidelines for conducting preventive examinations of employees set out in the Regulation of 30 May 1996 (Journal of Laws of 2023, item 607), as well as other examinations additionally ordered by the physician conducting the examination, in accordance with the applicable regulations, 2) conducting psychological examinations for the operation of powered industrial trucks used for internal transport at workplaces (forklift trucks), 3) conducting medical and psychological examinations of drivers operating vehicles up to 3.5 tonnes and over 3.5 tonnes; 4) conducting medical examinations for sanitary and epidemiological purposes; 5) issuing medical certificates stating that there are no health contraindications to work in a specified position or that there are health contraindications to work in a specified position, in accordance with the applicable templates, 6) monitoring employees’ health, in particular to identify possible occupational diseases as well as the causes of accidents at work, 7) initiating measures to protect and promote employees’ health and providing assistance with their implementation, 8) maintaining and storing medical documentation concerning preventive healthcare in accordance with the applicable regulations, 9) participation of an occupational medicine physician in the work of workplace occupational health and safety committees. 5. Examinations and the collection of material for testing will be performed at the Contractor’s facility/facilities. The Contracting Authority permits (in accordance with Article 14 of the Act of 27 June 1997 on occupational medicine services, consolidated text: Journal of Laws of 2014, 1184) certain diagnostic tests and specialist consultations to be performed at a Subcontractor’s facility, provided, however, that all tests and specialist consultations necessary to issue a certificate to an employee will be performed in one locality situated within the operating area of the relevant Regional Division of the Contracting Authority’s Human Resources and Payroll Office. At the same time, the Contracting Authority permits an Employee to be referred to a Contractor’s facility outside the area of the relevant Regional Division of the Human Resources and Payroll Office, provided that the Contractor’s facility in question is located closer to the Employee’s place of work. If the occupational medicine physician is absent, the Contractor undertakes to provide a replacement and to keep the Contracting Authority informed of this fact. 6. The Contractor undertakes to provide the service from Monday to Friday for at least 6 hours per day. This requirement also applies to subcontractors’ facilities. 7. The Contractor undertakes to perform preventive examinations and issue the medical certificate on the day when the employee or candidate for employment reports to the Contractor’s facility for examinations, with the proviso that the examinations must be completed within one day. 8. The Contractor undertakes to set the date for the preventive examinations on the day when the employee or candidate for employment reports the need for the examinations. The date will be: a) pre-employment examinations – no later than the 2nd working day following the date of notification, b) periodic examinations – no later than the 3rd working day following the date of notification, c) follow-up examinations – on the date of notification (in exceptional, justified situations, no later than the 2nd working day following the date of notification).
