A detailed description, scope and quantity of the subject matter of the contract and the contract performance conditions are contained in the pricing form for the subject matter of the contract constituting Annex No. 2 to the SWZ and the draft contract, Annex No. 4 to the SWZ, constituting an integral part of the SWZ. 3. The quantities specified in the pricing form are estimated/forecast values which, during the term of the contract, may be reduced depending on the current needs of the Contracting Authority. During the term of the contract, the Contracting Authority guarantees orders at a minimum level of 25% of the value of the concluded contract. 4. The Contractor shall offer a 60-day payment term from the date of issuance of a correct VAT invoice and shall offer a period of unchanged prices for the term of the contract. 5. If the Contractor finds that the content of the SWZ (including the annexes), within the scope concerning the description of the subject matter of the contract, refers to a trademark, patent or origin, source or particular process that characterises a specific product, it shall be deemed that such indication is accompanied by the words: “or equivalent”. The Contracting Authority shall consider as equivalent an assortment that has the same or better technical and quality parameters than those described in the SWZ, and whose application will not adversely affect the proper functioning/use of the assortment in accordance with its intended purpose. A Contractor relying on solutions equivalent to those described by the Contracting Authority is obliged to demonstrate that the supplies it offers meet the requirements specified by the Contracting Authority and will not result in additional costs arising from their use. In every case where standards, technical assessments, technical specifications and technical reference systems referred to in Article 101(1)(2) and (3) of the Public Procurement Law are used in the description of the subject matter of the contract, the Contractor should assume that such reference is accompanied by the words “or equivalent”. successive purchase of stents, balloon catheters, introducers and others.
