Opportunity description
1. The subject matter of the contract is the supply of specialized sterile medical devices used in the Department of Endoscopic Diagnostics and Therapy and in the Central Operating Suite. 2. A detailed description and the manner of performance of the contract are contained in the OPZ, constituting Annexes No. 1-29 to the SWZ. The Contractor may submit a tender for any number of tasks. 3. Where the documentation indicates trademarks, patents or origins, sources or a specific process that characterizes the products, the Contracting Authority permits equivalent solutions to be offered. In the description of the subject matter of the contract, the Contracting Authority specified the criteria used to assess equivalence. 4. The Contracting Authority requires the offered subject matter of the contract to meet the following conditions (if applicable to its specific nature): a) each item must be packaged separately (individually); b) each individual package must contain a description in Polish; c) each individual package must bear a permanent and legible expiry date and serial number; d) it must be sterile, which requires clear labeling on the unit package; e) each individual package must contain instructions for use in Polish. 5. The Contracting Authority requires that: the expiry period for the supplied subject matter of the contract be min. 12 months from the date of delivery of the goods to the Contracting Authority’s premises. 1.
