Opportunity description

1. The subject of the contract is the supply of orthopaedic implants for the John Paul II Provincial Hospital in Bełchatów. 2. The description, quantity and scope of the subject of the contract are set out in Appendix No. 7 to the SWZ. 1) The contracting authority divided the subject of the contract into 38 parts (packages 1 to 38), in accordance with the description provided in Chapter XXIII of the SWZ. 2) The subject of the contract offered in the procedure has all the necessary certificates, including declarations of conformity, complies with the standards applicable within the European Union, meets the requirements specified in the Act of 7 April 2022 on medical devices, and has the authorisations for marketing and use required by superior legislation 3) The remaining requirements concerning performance of the contract and the rules of cooperation between the contracting authority and the contractor are specified in the draft provisions of the public procurement contract, constituting Appendix No. 3 to the SWZ.
