Opportunity description

1. The subject matter of the contract is the supply of orthopaedic implants and consumables. 2. A detailed description of the subject matter of the contract, together with the required parameters and quantitative demand, is specified in Annex No. 2 to the SWZ – Price Form. 3. The offered subject matter of the contract must comply with the Contracting Authority's requirements described in the SWZ and with the laws applicable in Poland. Furthermore, the subject matter of the contract must bear the CE marking and have declarations of conformity, approvals, certificates, and registration certificates if required by law. Medical devices must additionally have certificates authorising their placing on the market and use in Polish healthcare facilities. The Contractor undertakes to provide the above documents upon each request of the Contracting Authority. 4. The offered subject matter of the contract must have the appropriate authorisation for placing on the market and use in accordance with the Act of 7 April 2022 on medical devices and the Act of 18 March 2011 on the Office for Registration of Medicinal Products, Medical Devices and Biocidal Products.
