Opportunity description

1. The subject matter of the contract is the supply of pharmaceutical products divided into 104 lots for the Władysław Biegański District Hospital in Iława 2. Contractor's obligations: • Delivery and transport • Unloading and carrying into storage rooms. 2.1. Pharmaceutical products must have a shelf life of at least 12 months from the time of their delivery to the Contracting Authority's premises. Products with a shorter expiry date may be delivered only with the approval, on each occasion, of the Contracting Authority's authorized representative. 2.2. In each of the packages, the use of equivalent medicinal products is permitted, i.e. a preparation with an identical composition of active substances, dosage and pharmaceutical form (vial, ampoule, aerosol, dry substance, etc.). The preparation must have indications for use identical to those of the preparation specified in the subject matter specification. 2.3. Where the offered medicinal product package contains a different number of units (tablets, ampoules, etc.), the Contractor is obliged to recalculate the number of packages. If the recalculation results in a non-whole number of packages, the offered quantity must be rounded up. 2.4. If the offered preparation is unavailable on the market, the Contractor is obliged to replace it with a synonymous medicinal product at a price not higher than the tender price (after consultation with the Head of the Hospital Pharmacy Department). If the package of the synonymous medicinal product contains a different number of units (e.g. tablets, ampoules, etc.), its price must be recalculated so that the price of 1 unit (tablet, ampoule, etc.) in the package is not higher than the price of 1 unit under the tender contract. 2.5. If supplying an equivalent preparation is not possible or it is impossible to supply the missing medicinal product within 48 hours, the Contractor is obliged to issue a written note (which will constitute the basis for purchasing the medicinal product from another pharmaceutical wholesaler) and to cover the difference in purchase costs. 2.6. Transport packaging for medicinal products requiring storage in a cool place (2–8 degrees C) should be marked, and a single-use temperature indicator must be placed inside the packaging to enable confirmation of appropriate transport conditions. 2.7. Where, during the tender procedure, the medicinal product required by the Contracting Authority is not being manufactured, the price form constituting Appendix No. 1 to the SWZ must state the last sale price and the appropriate information must be indicated. 2.8. It is permitted to change the pharmaceutical form of the medicinal product from: -capsules to tablets, from tablets to film-coated tablets or capsules and vice versa - vials to ampoules or pre-filled syringes and vice versa A detailed description is contained in Appendix No. 2, Assortment and Price Form Procedure: open.
