Opportunity description

1. The subject matter of the contract is the supply of pharmaceutical products for the needs of the Hospital Departments of the Międzyleski Specialist Hospital in Warsaw. 2. The subject matter of the contract has been divided into 24 task groups, in the quantities and assortment specified in the Pricing Forms constituting integral parts of this SWZ. 3. The tender may concern all or selected Tasks. Partial tenders within individual Tasks are not permitted. 4. The Contracting Authority does not permit variant tenders. 5. Each Contractor has the right to submit only one tender. The submission by one Contractor of more than one tender or a tender containing alternatives will result in the rejection of all tenders submitted by that Contractor. 6. The assortment offered in the tender procedure (listed in the Pricing Forms) must comply with the description of the subject matter of the contract and must meet the conditions for authorization for use in healthcare facilities in accordance with applicable law. 7. Failure to meet any of the required parameters gives the Contracting Authority grounds to reject the tender. 8. The contract will be performed for a period of 12 months from the date of conclusion of a valid agreement with the Contractor selected as a result of the public procurement procedure. 9. Tenders will be evaluated on the basis of the criteria described in Chapter XV of this SWZ. 10. Delivery time to the warehouse of the Contracting Authority's Hospital Pharmacy - no longer than 1 day. 11. The draft contractual provisions constitute Annex No. 1 to the SWZ, which is an integral part of this documentation. The provisions contained in the SWZ shall be treated as the conditions for awarding the contract. 12. Subcontracting 1. The Contractor may entrust the performance of part of the contract to subcontractors, 2. The Contractor is required to indicate in the tender the parts of the contract whose performance it intends to entrust to subcontractors, specifying the subcontractors' companies. 3. The Contracting Authority informs that it does not require the Contractor to personally perform key parts of the contract. 4. If the Contracting Authority determines that grounds for exclusion apply to a given subcontractor, the Contractor is required to replace that subcontractor or refrain from entrusting the performance of part of the contract to the subcontractor. 5. Entrusting the performance of part of the contract to subcontractors does not release the Contractor from liability for the proper performance of this contract.
