Opportunity description
The subject of the contract is the successive supply of medicines and medical devices in the assortments and quantities specified in the price form constituting Appendix No. 2 to the SWZ and in accordance with the draft contractual provisions constituting Appendix No. 4 to this SWZ. In Appendix No. 2 to the SWZ, the Contracting Authority specified quality requirements relating at least to the main elements comprising the subject of the contract. The Contracting Authority guarantees that the minimum value of supplies during the term of the contract within the "basic scope" will amount to 75 % of the maximum contract value within the "basic scope" specified in § 5 of the contract for the given task. - does not apply to task No. 9, 10, 36; the provision "basic scope" applies to task No. 9, 29. The goods offered by the Contractor and covered by the subject of the contract must be authorized for marketing and use in Poland or must have a one-time authorization from the Ministry of Health. The Contractor should offer and price all assortment items listed in the price form. In the price form, the Contractor should provide the trade name of the goods offered and the EAN code. . The prices of the products offered may not exceed the limits of their reimbursement announced in the notices of the Minister of Health concerning the list of reimbursed medicines, foodstuffs intended for particular nutritional uses, and medical devices. - applies to items included in the list of reimbursed medicines. If a given medicine is removed from the list of reimbursed medicines or if its production is discontinued, its marketing authorization expires, or the medicine is suspended or withdrawn from the market, the Contractor will be obliged to offer the Contracting Authority a medicine with the same international nonproprietary name (if any), dose, form, and indications, included in the list of reimbursed medicines, at the lowest price among the remaining medicines, provided that this price does not exceed the reimbursement limit, subject to § 6 para.3 point 5 of Appendix No. 4 to the SWZ.- applies to items included in the list of reimbursed medicines. The Contracting Authority hereby informs that, in the present procedure conducted under the open tender procedure, it provides for the possibility of applying the procedure referred to in Art. 139 para. 1 of the Public Procurement Law. When applying the aforementioned procedure, the Contracting Authority will first examine and evaluate the tenders and then carry out the qualification of the Contractor whose tender received the highest evaluation with regard to the absence of grounds for exclusion and fulfillment of the conditions for participation in the procedure. Task No. 1 – Supplies of medicines from therapeutic programs The subject of the contract is the successive supply of medicines and medical devices in the assortments and quantities specified in the price form constituting Appendix No. 2 to the SWZ and in accordance with the draft contractual provisions constituting Appendix No. 4 to this SWZ. Task No. 3 – Supplies of medicines from therapeutic programs The subject of the contract is the successive supply of medicines and medical devices in the assortments and quantities specified in the price form constituting Appendix No. 2 to the SWZ and in accordance with the draft contractual provisions constituting Appendix No. 4 to this SWZ. Task No. 9– Supplies of medicines from therapeutic programs The subject of the contract is the successive supply of medicines and medical devices in the assortments and quantities specified in the price form constituting Appendix No. 2 to the SWZ and in accordance with the draft contractual provisions constituting Appendix No. 4 to this SWZ. Task No. 14– Supplies of infusion fluids The subject of the contract is the successive supply of medicines and medical devices in the assortments and quantities specified in the price form constituting Appendix No. 2 to the SWZ and in accordance with the draft contractual provisions constituting Appendix No. 4 to this SWZ. Task No. 19 – Supplies of various medicines The subject of the contract is the successive supply of medicines and medical devices in the assortments and quantities specified in the price form constituting Appendix No. 2 to the SWZ and in accordance with the draft contractual provisions constituting Appendix No. 4 to this SWZ. Task No. 28 – Supplies of various medicines The subject of the contract is the successive supply of medicines and medical devices in the assortments and quantities specified in the price form constituting Appendix No. 2 to the SWZ and in accordance with the draft contractual provisions constituting Appendix No. 4 to this SWZ. Task No. 36 – Supplies of various medicines The subject of the contract is the successive supply of medicines and medical devices in the assortments and quantities specified in the price form constituting Appendix No. 2 to the SWZ and in accordance with the draft contractual provisions constituting Appendix No. 4 to this SWZ.
