The subject matter of the contract is the supply of various medicines under 14 packages to the Contracting Authority's premises, in accordance with the description and detailed requirements contained in Annex No. 2 to the SWZ.The procedure is conducted via the e-Zamówienia Platform. Documents comprising the tender: a) Tender Form (Annex No. 1 to the SWZ), made available by the Contracting Authority on the e-Zamówienia Platform and included in the procedure preview under the “Basic information” tab, signed with a qualified electronic signature by persons duly authorised to represent the Contractor, b) detailed price tender (Annex No. 2 to the SWZ) NOTE! If the Detailed Price Tender is not attached to the Tender, the Contracting Authority will reject the Contractor's tender, c) documents and declarations confirming that the Contractor meets the conditions for participation in the Procedure and that there are no grounds for exclusion (listed in Chapter X.1 of the Specification), d) documents indicated in Chapter XI of the Specification (if applicable), e) in the case of Contractors acting through an authorised representative – Power of Attorney, f) in the case of Contractors jointly applying for the contract – a document confirming the appointment by the Contractors jointly applying for the contract of an authorised representative to represent them in the contract award Procedure or to represent them in the Procedure and conclude the public procurement contract, g) confirmation that the tender security has been provided. In accordance with Article 139(1) of the Pzp Act, the Contracting Authority informs that it will first examine and evaluate the tenders and then carry out the entity qualification of the Contractor whose tender was rated highest, with regard to the absence of grounds for exclusion and fulfilment of the conditions for participation in the procedure.
