The subject of the contract is the provision of transport services (transport to and from) for dialysis patients from their place of residence to the Dialysis Station of the County Hospital in Chrzanów and back. Transport will be carried out using the Contractor’s vehicles in a group system, taking into account the patients’ place of residence, health condition, and mobility. The service will be provided in accordance with the schedule (Dialysis Plan) approved by the Contracting Authority. The Contractor undertakes to provide punctual transport, ensuring the uninterrupted operation of the Dialysis Station.
