The subject matter of the contract is healthcare services to be provided within the Pulmonary Diseases Department with the Chemotherapy Subdepartment of Szpital Miejski w Rudzie Śląskiej Sp. z o.o. The term “healthcare services” shall mean all activities aimed at maintaining, saving, restoring or improving health, as well as other medical activities arising from the treatment process or separate provisions governing the rules for their performance. 2. The subject matter of the contract has been divided into V packages: Package I – provision of healthcare services within the Pulmonary Diseases Department with the Chemotherapy Subdepartment of Szpital Miejski w Rudzie Śląskiej Sp. z o.o., in accordance with a schedule approved by the Head of the Department/Deputy Head of the Department/Medical Director: 1) from Monday to Friday between 07:25 and 15:00 (an average of 4 services per month); 2) from Monday to Friday between 15:00 and 07:25 on the following day (an average of 4 services per month); 3) on Saturdays, Sundays and statutory non-working days (public holidays) between 07:25 and 07:25 on the following day (an average of 1 service per month); The female/male physician providing the services must hold the title of specialist in pulmonary diseases. Under Package I, the Contracting Authority permits the selection of 1 (one) most advantageous tender that meets all conditions required by the Contracting Authority. Package II – provision of healthcare services within the Pulmonary Diseases Department with the Chemotherapy Subdepartment of Szpital Miejski w Rudzie Śląskiej Sp. z o.o., in accordance with a schedule approved by the Head of the Department/Deputy Head of the Department/Medical Director: 1) from Monday to Friday between 07:25 and 15:00 (an average of 21 services per month); 2) from Monday to Friday between 15:00 and 07:25 on the following day (an average of 3 services per month); 3) on Saturdays, Sundays and statutory non-working days (public holidays) between 07:25 and 07:25 on the following day (an average of 2 services per month); The female/male physician providing the services must hold the title of specialist in pulmonary diseases or internal medicine. Under Package II, the Contracting Authority permits the selection of a maximum of 2 (two) most advantageous tenders that meet all conditions required by the Contracting Authority. Under Package V, the Contracting Authority permits the selection of 1 (one) most advantageous tender that meets all conditions required by the Contracting Authority. 3. The detailed work arrangements are specified by the Department’s work schedule applicable at the Contracting Authority and prepared in advance for each month. The Contracting Authority reserves the right to amend the schedule, but no later than 3 days before the beginning of the given month. The schedule is approved by the Head of the Department/Deputy Head of the Department/Medical Director. 4. In accordance with the Act of 13.05.2016 on counteracting threats of sexual crime, during the examination of tenders the Contracting Authority will verify whether the Tenderer is listed in the Register of Sexual Offenders. If listed in such a register, the tender of the Contract Recipient will be rejected. 5. The standards for the protection of minors are available on the Hospital’s website at: www.szpitalruda.pl 6.
