Task name: “Service for the collection and management of municipal waste generated on properties in the Łazy municipality where residents reside”. The Contracting Authority divides the subject matter of the contract into the following tasks: Task 1 – “Service for the collection and management of waste generated ‘at source’ from inhabited properties, expired medicines and batteries from collection points, and collection of waste from the PSZOK”. Task 2 – Mobile collection and management service for bulky waste, including waste electrical and electronic equipment, generated on properties within the Łazy municipality. A detailed description of the subject matter of the contract is contained in Appendix No. 9 to the SWZ. The Contracting Authority informs that it will additionally publish the procedure on its website: https://bip.umlazy.finn.pl The subject matter of the contract is the provision of a service for the collection and management of municipal waste originating from inhabited properties within the Łazy municipality. Under the subject matter of the contract, the Contractor is obliged to provide the following services to the Contracting Authority: 1. Provision of a service for the collection and management of municipal waste generated on inhabited properties within the Łazy Municipality 2. Provision of containers for unsorted (mixed) municipal waste in multi-unit buildings, 3. Provision of containers (in multi-unit buildings) and delivery of bags (to single-family properties) for selectively collected waste 4. Maintenance of all municipal waste containers, including containers for biowaste, expired and unused medicines, and used batteries, in proper sanitary, orderly and technical condition 5. Monitoring compliance with the obligation to collect waste selectively 6. Responding to complaints and liability for damage 7. Collection and management of waste from the Selective Waste Collection Point at ul. Pocztowa 14 in Łazy, as well as collection of waste from pharmacies and used-battery collection points. 8. Reporting by the Contractor to the Contracting Authority 9. Other services covered by the subject matter of the contract indicated in this SWZ and in the appendices. The Contracting Authority requires that the persons whom the contractor will use in performing the services constituting the subject matter of the contract (within the scope of Task No. 1 and Task No. 2) be employed under an employment contract throughout the entire term of the contract. The Contracting Authority requires the contractor or subcontractor to employ under an employment contract persons performing the following activities in connection with performance of the contract: a) driving vehicles intended for performance of the contract; b) loading collected municipal waste; c) supervising the performance of services covered by the subject matter of the contract. To confirm compliance with this requirement, before signing the contract the contractor shall submit a declaration—in accordance with the provisions of Appendix No. 7 to the SWZ. The Contracting Authority provides for the award of contracts referred to in Art. 214(1)(7) of the Pzp in the amount of 20% of the contract value (for each task). The contract will consist of performing the services indicated in the price form for this procedure.
