1. The subject matter of the contract is the provision of transport services for students with disabilities from the Municipality of Kobierzyce to educational and care institutions and schools and their places of residence in the school years: 2026/2027 and 2027/2028 2. Designation of the subject matter of the contract in the Common Procurement Vocabulary (CPV): Main subject: 60130000-8 Special-purpose road passenger-transport services 3. Description of the Subject Matter of the Contract: 1) Provision of transport services for students with disabilities from the Municipality of Kobierzyce to educational and care institutions and schools and their places of residence in the school years: 2026/2027 and 2027/2028. a) The service consists of the Contractor providing transport services for students with disabilities subject to compulsory schooling and one-year compulsory pre-school preparation, b) The service also includes the provision of care for students while they are being transported by the Contractor, c) Transport of students from their places of residence to educational and care institutions and schools and from educational and care institutions and schools to the students’ places of residence will take place only on days when educational units perform teaching, educational and care tasks, d) The Contractor will provide transport services on school days, excluding holiday breaks and other days free from classes RZP.271.39.2026 Page 5, in accordance with the organization of the school year published by the MEN for the given school year. e) The service will be provided on all days of school classes in accordance with the calendar for the 2026/2027 and 2027/2028 school years, from 01.09.2026 to 23.06.2028, between 6.00 and 17.00. 2) The estimated average daily number of kilometers (transport to and from all institutions) is approximately 890 km. 3) The projected list of the number of children and the centers and schools to which children with disabilities will be transported, as well as their places of residence, constitutes Appendix No. 1 to the SOPZ (Appendix A to the SWZ). 4) Transport will be carried out along specified routes. The projected routes constitute Appendix No. 2 to the SOPZ (Appendix A to the SWZ). 4. The Detailed Description of the Subject Matter of the Contract (SOPZ) is contained in Appendix A to the SWZ, specifying the detailed scope and conditions for performance of the contract. 1.
