1. The subject matter of the contract is the supply to the Provincial Hospital in Bielsko-Biała at Al. Armii Krajowej 101 of: 1) lenses, viscoelastic fluids, sets for cataract surgery and posterior vitrectomy, together with the loan of a device for phacoemulsification and anterior vitrectomy, a device for posterior vitrectomy and phacoemulsification, and an optical biometer – Part 1; 2) accessories for anti-glaucoma surgery, together with the loan of a device for anti-glaucoma surgery – Part 2; 3) intraocular lenses – Part 3; 4) additional accessories for posterior vitrectomy – Part 4; 5) additional accessories for posterior vitrectomy – Part 5; 6) additional accessories for posterior vitrectomy – Part 6; 7) additional accessories for posterior vitrectomy – Part 7; 8) additional accessories for ophthalmic surgery – Part 8; 9) additional accessories for ophthalmic surgery – Part 9; 10) additional accessories for ophthalmic surgery – Part 10; according to the quantitative and product-type breakdown specified in Annex No. 2 to the Specification of Contract Conditions, hereinafter referred to as the “swz”, meeting the quality parameters specified in Annex No. 2 to the swz, under the terms specified in the draft contractual provisions constituting, for Parts 1-2 – Annex No. 5a to the swz, and for Parts 3-10 – Annex No. 5b to the swz. 2. The goods offered must be new and unused (except for the loaned equipment for eye surgery). 3. Delivery of the subject matter of the contract, including transport and unloading, shall be carried out at the Contractor’s cost and risk. 4. Applies to Part 1 – For the duration of the contract, as part of the price of the subject matter of the contract with respect to Part 1, the Contractor shall provide, together with the first delivery of lenses, nine reusable implantation sets consisting of an injector and forceps for implanting the lenses specified in item 1, and two sets for marking the eye during the implantation of toric lenses. 5. Applies to Part 1 – For the duration of the contract, as part of the price of the subject matter of the contract with respect to Part 1, the Contractor shall provide and deposit at the Contracting Authority’s premises, within 7 weeks from the date of conclusion of the contract, a device for phacoemulsification and anterior vitrectomy, a device for posterior vitrectomy, and an optical biometer with the parameters and accessories specified in the table below, and shall ensure the proper functioning of the devices throughout the term of the contract. All operating costs of the devices, including inspections, repairs, maintenance and servicing, as well as training the Contracting Authority’s personnel in the operation of the devices, must be included in the price of the subject matter of the contract with respect to Part 1. 6. Applies to Part 2 – For the duration of the contract, as part of the price of the subject matter of the contract with respect to Part 2, the Contractor shall provide and deposit at the Contracting Authority’s premises, within 4 weeks from the date of conclusion of the contract, a device for anti-glaucoma surgery with the parameters and accessories specified in the table below, and shall ensure the proper functioning of the device throughout the term of the contract. All operating costs of the device, including inspections, repairs, maintenance and servicing, as well as training the Contracting Authority’s personnel in the operation of the device, must be included in the price of the subject matter of the contract with respect to Part 2. 1.
