1. Subject matter and scope of the contract: The subject matter of the contract is to conduct a nationwide, quantitative, field survey concerning health and risk factors among adult residents of Poland, using a questionnaire prepared by the Contracting Authority. The thematic scope of the study includes in particular: general health status, self-assessment, occurrence of chronic diseases and pain complaints, mental condition and sleep quality, accidents and injuries, lifestyle and risk factors: use of traditional tobacco products and their substitutes (including e-cigarettes/heated tobacco products), physical activity, consumption of alcohol and psychoactive substances, anthropometric measurements (BMI, body weight, height) and dietary habits, environmental conditions and harmful factors in the workplace, use of healthcare services (public services under the NFZ and private services), accessibility of POZ and specialist doctors, barriers, delays and abandonment of treatment, health prevention, preventive vaccinations and attitudes toward vaccination, well-being, social support, sense of exclusion, and the economic and demographic situation. The research tool is a questionnaire consisting mainly of closed-ended questions (single/multiple choice) and optionally 1–2 open-ended questions. The estimated average duration of one interview is approximately 35 minutes. 2. Methodology and sample selection Research technique: computer-assisted face-to-face questionnaire interviews (CAPI – Computer-Assisted Personal Interviewing). Study population: residents of Poland aged 18 and over, living in private households and speaking Polish. Sample size: N = 3,000 respondents. Sample structure: a representative nationwide random-quota sample divided by sex (1,500 women, 1,500 men) and age groups: 18–39 years (800 persons), 40–59 years (800 persons), 60–74 years (800 persons), 75–84 years (400 persons), and 85+ years (200 persons). Sampling frame and sampling scheme: GUS address database; selection of starting points representative of voivodeships and locality size classes (agglomeration rural area, non-agglomeration rural area, town/city up to 50,000, 50,000–200,000, 200,000+); interviews conducted using the random route method (clusters of 5–7 interviews). Representativeness: The Contractor is required to ensure that the sample is representative in terms of age, sex, education, and place of residence (voivodeship and locality class). 3. Main obligations of the Contractor and final deliverables Preparatory stage: submission of any comments on the questionnaire (1 business day), submission of a detailed study schedule (within 3 business days), programming of an electronic test version of the CAPI questionnaire (within 5 business days), and conducting training for interviewers with the participation of the Contracting Authority. Fieldwork stage: conducting CAPI interviews together with the submission of progress reports every 10 business days (demographic and geographic structure, interview duration statistics). Final deliverables: a report summarizing the implementation of the full study, a tabular presentation of the results, and a dataset in CSV and SPSS formats with a TERC identifier (TERYT register) assigned to each respondent. 4. Contract performance deadlineThe deadline indicated in the Contractor’s tender – no longer than 55 business days from the date of conclusion of the contract and no later than December 15, 2026 (the deadline covers all phases of implementation, including procedures for the verification and acceptance of reports). The contract award procedure is conducted pursuant to Art. 132 of the Act of September 11, 2019, Public Procurement Law. The Contracting Authority, acting pursuant to Art. 139 of the Pzp Act, will first examine and evaluate the tenders and will then carry out the qualitative selection of the Contractor whose tender was rated highest, with regard to the absence of grounds for exclusion and fulfillment of the conditions for participation in the procedure.
