Opportunity description

1. The subject of the contract is the purchase and delivery of a syringe-free automatic injector for computed tomography, including installation and assembly of the device, for the needs of the Katowice Oncology Center, in accordance with Appendix No. 2 to the SWZ – Price Offer. 2. The technical and functional parameters of the device are specified in Appendix No. 4 to the SWZ. The contract also includes: 1) transport of the device to the destination, 2) installation and assembly, 3) the cost of annual service inspections during the warranty period, 4) delivery within 4 weeks from the date of conclusion of the contract, 5) a 36-month warranty calculated from the date of signing by authorized representatives of the Contracting Authority and the Contractor of the “Final Acceptance Protocol for the Subject of the Contract”. 6) Under the warranty, the Contractor shall ensure, as part of the offer price, the operability of the device, including: • service inspections together with all parts necessary to perform the inspections, in accordance with the manufacturer’s requirements • all repairs, inspections, and maintenance, together with spare parts and materials needed to perform them, • repair and replacement of parts with new ones, except where the damage is caused by improper operation of the device, • extension of the warranty by the period during which the subject of the contract is inoperative in the event of warranty repairs, • commencement of repairs within 24 hours of reporting a defect, from Monday to Friday, excluding statutory public holidays, • repair time without the need to import parts from abroad: max. 3 days, from Monday to Friday, excluding statutory public holidays, • repair time requiring parts to be imported from abroad: max. 8 days, from Monday to Friday, excluding statutory public holidays, • the total inoperability* of the device (in the event of a defect) shall not exceed 20 working days in 12-month periods * inoperability means a lack of operability of the device preventing the performance of all procedures using it for more than 5 hours within a 24-hour period, including defects referred to in Appendix No. 4 – Technical and Functional Parameters. 7) delivery of a brand-new, unused device, with the year of manufacture no earlier than 2025.
