Opportunity description

1. The subject of the contract is the provision of medical transport services for patients, transport of biological materials, and transport of blood and its components for the Fryderyk Chopin University Clinical Hospital in Rzeszów at ul. Szopena 2 and ul. Rycerska 2. 2. A detailed description of the subject of the contract is included in the Description of the Subject of the Contract (Appendix No. 2 to the SWZ) and in the Price and Tender Form (Appendix No. 1 to the SWZ), which constitute an integral part of the SWZ. 3. The scope of the contract covers the provision of services for the above-mentioned subjects of the contract for a period of 36 months. 4. The Contracting Authority permits the submission of equivalent tenders. 5. CPC Cat.: 2. 6. CONTRACT PERFORMANCE PERIOD: Contract performance period: for 36 months from the date of conclusion of the contract. The Contracting Authority permits the possibility of changing the term of the contract if the entire subject of the contract is not completed during the contract period due to the fulfilment of lower-value orders—the change to the contract requires an annex, provided that the Contracting Authority unilaterally notifies the Contractor of the proposed extension of the contract before the expiry of the period for which it was concluded.
