Komunalna 5, 75-724 Koszalin, between 7.00 and 14.00 from Monday to Friday, except on public holidays, after notifying the Contracting Authority at least 3 working days before delivery. The delivered vehicle must have the appropriate documents allowing it to be registered with the competent authorities. The delivered body must have the appropriate technical documents, including a UDT decision. SUBJECT-MATTER EVIDENCE To confirm that the offered supply complies with the requirements specified in Chapter II of the SWZ – Detailed description of the subject matter of the contract, the Contracting Authority requires the following subject-matter evidence to be submitted together with the tender: 1. The subject-matter evidence consists of information about the offered subject matter of the contract (vehicle on a three-axle chassis, with a load compartment capacity of at least 21 m3, equipped with a weighing system, RFID tag reader, GPS and video monitoring. 2. Together with the “Tender form”, the Contractor shall submit completed Annex No. 2 to Chapter I of the SWZ – “Information about the offered subject matter of the contract”.3. If Annex No. 2 to Chapter I of the SWZ – “Information about the offered subject matter of the contract” is not submitted or individual information is missing from it, the Contracting Authority provides for the submission or supplementation of this document within a specified time limit.7. FOREIGN CONTRACTORS. 1) If the Contractor has its registered office or place of residence outside the territory of the Republic of Poland, instead of: 1.1) Information from the National Criminal Register referred to in Chapter I point 6.1 subpoint 3.1 of the SWZ – it shall submit information from the relevant register, such as a court register, or, if no such register exists, another equivalent document issued by the competent judicial or administrative authority of the country in which the Contractor has its registered office or place of residence or in which the person concerned by the information or document resides, within the scope referred to in Chapter I point 6.1 subpoint 3.1 of the SWZ; 1.2) A copy of or Information from the National Court Register or the Central Register and Information on Economic Activity, referred to in Chapter I point 6.1 subpoint 3.3 of the SWZ – it shall submit a document or documents issued in the country in which the Contractor has its registered office or place of residence, confirming that its liquidation has not been opened, it has not been declared bankrupt, its assets are not being managed by a liquidator or court, it has not entered into an arrangement with creditors, its business activity has not been suspended, and it is not in any other similar situation resulting from a similar procedure provided for under the laws of the place where that procedure was initiated. 2) The document referred to in subpoint 1.1 should have been issued no earlier than 6 months before its submission. The document referred to in point 1.2 should have been issued no earlier than 3 months before its submission. 3) If, in the country in which the Contractor has its registered office or place of residence or in which the person concerned by the document resides, the documents referred to in subpoint 1 are not issued, or if such documents do not refer to all the cases referred to in Article 108(1)(1), (2) and (4) of the PZP Act, they shall be replaced, as appropriate, in whole or in part, by a document containing, as appropriate, a declaration by the Contractor, indicating the person or persons authorised to represent it, or a declaration by the person whom the document was to concern, made under oath, or, if the country in which the Contractor has its registered office or place of residence or in which the person whom the document was to concern resides has no provisions on declarations made under oath, made before a judicial or administrative authority, a notary, or a competent professional or economic self-government body having jurisdiction over the registered office or place of residence of the Contractor or the place of residence of the person whom the document was to concern. The provision of subpoint 2 shall apply. 8.METHOD OF PREPARING ELECTRONIC DOCUMENTS. 1) The method of preparing electronic documents must comply with the requirements specified in the Regulation of the President of the Council of Ministers of 30 December 2020 on the method of preparing and transmitting information and the technical requirements for electronic documents and means of electronic communication in a public procurement or design contest procedure (Journal of Laws of 2020, item 2452) and the Regulation of the Minister of Development, Labour and Technology of 23 December 2020 on entity-related evidence and other documents or declarations that the Contracting Authority may require from the Contractor (Journal of Laws of 2020, item 2415, as amended).2) Entity-related evidence, subject-matter evidence and other documents or declarations prepared in a foreign language shall be submitted together with a translation into Polish. 3) If the Contractor indicates the availability of entity-related evidence referred to in Chapter I point 6.1 of the SWZ or documents referred to in Chapter I point 14 subpoint 11 of the SWZ at specified Internet addresses of publicly accessible and free databases, the Contracting Authority requires the Contractor to provide a translation into Polish of the entity-related evidence or documents independently retrieved by the Contracting Authority. 4) JEDZ: 4.1) The Contractor shall prepare the JEDZ declaration using the service available through the website https://espd.uzp.gov.pl/ or through other available tools or software that enable completion of the JEDZ and creation of an electronic document.
