I. Subject of the supply. Supply of a vehicle with a permissible gross weight of up to 3.5 Mg with a lift for the collection of municipal waste. 1. The price must be expressed in PLN. 2. The price must include all delivery costs, e.g. excise duty, customs duty, taxes, etc. transport, insurance until delivery to the place indicated by the Contracting Authority,.3. The vehicle together with the body must be made of factory-new materials. 4. The vehicle must be delivered to the registered office of Przedsiębiorstwo Gospodarki Komunalnej Sp. z o.o., ul. Komunalna 5, 75-724 Koszalin between 7.00 and 14.00 from Monday to Friday, except for statutory public holidays, after notifying the Contracting Authority at least 3 working days before delivery. 5. The delivered vehicle must have the appropriate documents allowing it to be registered with the competent authorities. 6. The delivered lift must have the appropriate technical documents, including a UDT decision. II. Technical parameters of the vehicle – chassis.1. Factory-new.2. Unused, with a maximum mileage of up to 1,500 km.3. Year of manufacture; 2026 .4. Permissible gross vehicle weight (GVW): maximum 3.5 Mg , 5. Payload of the completed vehicle minimum 0.9 Mg(+/-10%).6. Height of the completed vehicle prepared for driving: maximum 4 m. The Detailed Description of the Subject Matter of the Contract is contained in Chapter II of the SWZ " Detailed description of the subject matter of the contract, anticipated annual mileage for the subject matter of the contract - 50,000 km per year, the Contracting Authority will permit a vehicle without rear sideboards if a lift is installed, Lift width adapted to the width of the body, i.e. minimum 2100 mm, Lift lifting height adapted to the height of the vehicle floor, The Contracting Authority requires standard mud flaps, the Contracting Authority will permit a vehicle with a power of 125 kW, The Contracting Authority confirms that it requires the tarpaulin to be divided into two parts with a frame post positioned halfway along the length of the body, The Contracting Authority permits a right-hand mirror on the passenger side divided into two mirrors, where each of the external mirrors consists of two parts: A large adjustable mirror is located at the top. Below it is a separate wide-angle mirror that can be adjusted to serve as a mirror, The Contracting Authority amends the provision in Chapter II point 7.10, subpoint 4), which shall read as follows: Light bar on the cab roof bearing the inscription “PGK Koszalin”, with orange LED warning lights. The Contracting Authority permits two flashing yellow warning lights mounted in the radiator grille, The Contracting Authority deletes the provision: " side covers of the bar" ' estimated annual vehicle mileage - 50,000 km per year, The Contracting Authority permits a vehicle without a right-hand kerb mirror and front “approach” mirror on the passenger side, Continuation of the text begun in the section " Information on the procedure" -grounds for exclusion, qualification-related evidence and other relevant information"...3.4) The Contractor's declaration that the information contained in the Declaration referred to in Chapter I point 6 of the SWZ (ESPD) remains current with respect to the grounds for exclusion from the procedure indicated by the Contracting Authority, referred to in: a) Article 108(1)(3) of the PZP Act, b) Article 108(1)(4) of the PZP Act, concerning the imposition of a prohibition on applying for a public contract as a preventive measure, c) Article 108(1)(5) of the PZP Act, concerning the conclusion of an agreement with other Contractors aimed at distorting competition, d) Article 108(1)(6) of the PZP Act. Note! 1. The Contractor is not required to submit qualification-related evidence that is held by the Contracting Authority if the Contractor identifies such evidence and confirms that it is correct and current. 2. The Contracting Authority will not request the submission of qualification-related evidence if: 2.1) it can obtain it using free and publicly available databases, in particular public registers within the meaning of the Act of 17 February 2005 on the computerisation of the activities of entities performing public tasks, provided that the Contractor has indicated in the Declaration referred to in Chapter I point 6 of the SWZ (ESPD) the data enabling access to such evidence; 2.2) the qualification-related evidence is a declaration whose content corresponds to the scope of the Declaration referred to in Chapter I point 6 of the SWZ (ESPD).SUBJECT-MATTER EVIDENCE - To confirm that the offered supply complies with the requirements specified in Chapter II of the SWZ – Detailed description of the subject matter of the contract, the Contracting Authority requires the submission, together with the tender, of the subject-matter evidence listed below: 1. The subject-matter evidence consists of information on the offered subject matter of the contract - a vehicle with a permissible gross weight of up to 3.5 Mg with a lift. 2. Together with the “Tender Form”, the Contractor shall submit completed Annex No. 2 to Chapter I of the SWZ – “Information on the offered subject matter of the contract”. 3. If Annex No. 2 to Chapter I of the SWZ – “Information on the offered subject matter of the contract” is not submitted or individual pieces of information are missing, the Contracting Authority provides for the submission or supplementation of this document within a specified time limit. 7. FOREIGN CONTRACTORS. 1) If the Contractor has its registered office or place of residence outside the borders of the Republic of Poland, instead of: 1.1) Information from the National Criminal Register referred to in Chapter I point 6.1 subpoint 3.1 of the SWZ – it shall submit information from the relevant register, such as a court register, or, if there is no such register, another equivalent document issued by the competent judicial or administrative authority of the country in which the Contractor has its registered office or place of residence, or in which the person concerned by the information or document has their place of residence, within the scope referred to in Chapter I point 6.1 subpoint 3.1 of the SWZ; 1.2) A copy of or Information from the National Court Register or from the Central Register and Information on Economic Activity referred to in Chapter I point 6.1 subpoint 3.3 of the SWZ – it shall submit a document or documents issued in the country in which the Contractor has its registered office or place of residence, confirming that its liquidation has not been opened, it has not been declared bankrupt, its assets are not managed by a liquidator or a court, it has not entered into an arrangement with creditors, its business activity is not suspended, and it is not in any other situation of this kind arising from a similar procedure provided for in the laws of the place where such procedure was initiated. 2) The document referred to in subpoint 1.1 should have been issued no earlier than 6 months before its submission. The document referred to in point 1.2 should have been issued no earlier than 3 months before its submission. 3) If, in the country in which the Contractor has its registered office or place of residence, or in which the person concerned by the document has their place of residence, the documents referred to in subpoint 1 are not issued, or if such documents do not cover all the cases referred to in Article 108(1), points 1, 2 and 4 of the PZP Act, they shall be replaced, as appropriate, in whole or in part by a document containing, as appropriate, a declaration by the Contractor, identifying the person or persons authorised to represent it, or a declaration by the person whom the document was to concern, made under oath, or, if in the country in which the Contractor has its registered office or place of residence, or in which the person whom the document was to concern has their place of residence, there are no provisions concerning declarations under oath, made before a judicial or administrative authority, a notary, or a professional or economic self-government body competent with regard to the registered office or place of residence of the Contractor or the place of residence of the person whom the document was to concern. The provision of subpoint 2 shall apply accordingly SUBCONTRACTORS. 1) The Contracting Authority, pursuant to Article 462(2) of the PZP Act, requires the Contractor to indicate – in point 12 of the Tender Form – the parts of the contract that it intends to entrust to subcontractors and to provide the names of any subcontractors, if already known. If the Contractor does not intend to entrust part of the contract to a subcontractor, the information in this point should be omitted or marked “not applicable”. 2) If a change or withdrawal of a subcontractor concerns an entity on whose resources the Contractor relied, under the rules set out in Chapter I point 5.1 subpoint 1 of the SWZ, in order to demonstrate fulfilment of the conditions for participation in the procedure, the Contractor is required to demonstrate to the Contracting Authority that the proposed other subcontractor or the Contractor itself fulfils them to a degree no less than the subcontractor on whose resources the Contractor relied during the public procurement procedure. The provision contained in Chapter I point 5.1 subpoint 6 of the SWZ shall apply accordingly. Procedure: open. Estimated value: 166,016.26 PLN.
